We’re looking for a Transition Support Worker to join our Children’s Team at Diverse Abilities.

We need you to help us to deliver exceptional, high-quality care for children living with disabilities.

*Due to the current demands on the service, we require a FEMALE Support Worker only*

Diverse Abilities is Dorset’s disability charity who have been supporting children and adults with profound physical and learning disabilities.

The people we support are unique. They are people living with profound and sometimes undiagnosed conditions. Their medical needs are complex, their disabilities are life limiting. Despite these disadvantages, we believe they should live the best life possible.

The role:

Hourly Rate: £12 per hour

Hours per week : 16 to 38 hours per week

  • Shifts covering 9am to 3.30pm Monday to Friday
  • You will be required to work alternate weekends ( You will have a day off in the week when working the weekend )

To ensure positive outcomes and the continual safety, health and wellbeing of the young people using Transition Services.

We are seeking a support worker for a 16 to 38 hours a week contract. Working on Shapes Transition and Lawford Lodge day care service for 18-25 yr olds who have profound and multiple learning disabilities as well as complex medical needs.

This role requires staff to

  • follow each individual young person’s care plan and contribute to their continuing development
  • assist where necessary all young people with intimate and personal care e.g. personal hygiene, feminine hygiene and bathing
  • assist where necessary any young person with eating and drinking or maintaining their nutritional needs
  • encourage, so far as possible the young people to high standards of cleanliness and tidiness
  • support young people to fulfil their social and emotional needs through play/activities

It is a requirement that applicants have, are working towards or are willing to complete a relevant level three diploma.

You will be expected to have an understanding of the Children’s Act, Care Act and the minimum Care Standards as well as demonstrating an understanding and experience of working with people who have complex needs.

You will need to adhere to the highest standards of Child and Adult Protection Procedures.

What we need from you:

Care experience and/or childcare experience.

**Full UK Manual driving licence**

Flexible working over 7 days.
