Hybrid role - some remote working and some onsite working at a later date

Pay: Self employed £45 per hour

PAYE employed: £35 per hour

The Advanced Practitioner (AP) will lead and support the end-to-end delivery of the Lipids Programme, focusing on the identification, assessment, treatment, prescribing, and ongoing management of patients requiring lipid optimisation.

The postholder will independently manage patients within agreed clinical pathways and will support the initiation, prescribing, administration, monitoring, and ongoing management lipid lowering therapies in line with NICE guidance, local pathways, and national cardiovascular prevention priorities.

The role will work collaboratively across primary care, neighbourhood teams, and wider healthcare partners to improve cardiovascular prevention outcomes and support proactive population health management.

Main duties of the job

Clinical Responsibilities

  • Independently assess, diagnose, and manage patients needing lipid optimisation.
  • Identify and review those with hyperlipidaemia, familial hypercholesterolaemia (FH), established cardiovascular disease (CVD), or elevated risk.
  • Support the full lipid management pathway: patient identification, clinical assessment, risk stratification, medication initiation and optimisation, and ongoing monitoring and review.
  • Prescribe lipid-lowering therapies (oral and injectable) in line with competence, NICE guidance, and local pathways.
  • Monitor treatment response, safety, adherence, and outcomes; interpret investigations to inform decisions; and refer complex cases when required.
  • Maintain accurate clinical records.

Patient Education & Prevention

  • Provide expert advice on cholesterol management, CVD prevention, lifestyle modification, diet, exercise, smoking cessation, and medication adherence.
  • Promote shared decision-making and personalised care, improving patient understanding of risk and treatment benefits.

Service Delivery & Leadership

  • Support delivery of the Lipids programme across practices and neighbourhood teams.
  • Contribute to pathway development, governance, and service improvement.
  • Participate in MDT working across primary and secondary care, support audit and evaluation, and act as a clinical lipid management resource within the multidisciplinary team.
