Job Description

Job Title: Advanced Nurse Practitioner

Reporting to: The Clinical Director and Business Manager

Salary: Depending on experience

Hours: To be discussed - 9 month fixed term contract

Overall Purpose

Wimborne and Ferndown PCN are looking for an enthusiastic, friendly Advanced Nurse Practitioner to join our PCN team. The successful applicant will work closely with our core PCN team as well as practice teams to deliver care to our patients. Quarter Jack Surgery have a team of 5 Nurse Practitioners who would work closely with and support our new PCN Advanced Nurse Practitioner.

Our Primary Care Network (PCN) comprises 5 practices in Wimborne (Quarter Jack Surgery, Walford Mill, Old Dispensary) and Ferndown (Penny’s Hill and Orchid House). The population size is 42,000. Each of the member practices have a good reputation, are well organised, consistently perform well in their Quality and Outcome Frameworks and at their last CQC inspections performed ‘good’. Our PCN is committed to delivering high quality care.

Job Summary:

The post-holder will manage a caseload and deal with a wide range of health needs in a
primary care setting, ensuring the highest standards of care for all registered and temporary patients.

Clinical Responsibilities:

In accordance with the PCN timetable, the post-holder will:

  • act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate in a variety of appointment types (face to face, telephone, video.)
  • Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
  • Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services.
  • Assesses diagnoses and treat patients attending primary health care services with a range of acute, non-acute and chronic medical conditions.
  • Assess diagnoses and treat patients in surgery who require acute medical attention and refer appropriately in line with local and national guidance.
  • Admit patients to secondary care for acute medical need.
  • Refers patient as appropriate to other members of the multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray in line with policy and appropriateness.
  • Order blood and other pathology diagnostic testing via ICE.
  • Make critical judgements to satisfy the expectations and clinical need of the patient, where appropriate.
  • Work within professional guidelines and codes of conduct.
  • Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.
  • Prescribes independently but in line with the prescribing formulary.

Maintaining, monitoring and updating computer database and management of medical records and practice audits

The post holder will:

  • Ensure that the clinical system (SystmOne) is updated promptly and accurately with information relating to patient records, as required for clinical accuracy and for practice and PCN recording requirements under QOF, IIF and Enhanced Services, utilising Ardens templates where appropriate.
  • Use the appropriate read and SNOMED codes.
  • Participate in the collection and collation of statistics towards the preparation of PCN reports, performance monitoring and audits.
  • Participate in the auditing of PCN activity.

PCN Organisation

The post holder will:

  • Attend any PCN meetings, significant event audit meetings and others as deemed necessary
  • Ensure that the PCN complaints system is adhered to at all times
  • Have an awareness and compliance with all relevant guidelines e.g. data protection, confidentiality and health and safety
  • Maintain current CPR and anaphylaxis certification
  • Ensure safeguarding training, accreditation and pathways are adhered to at all times.

Confidentiality

In the course of seeking treatment, patients entrust us with private, sensitive information in relation to their health and other matters in confidence and have the right to expect that staff will respect their privacy and uphold their right of confidentiality at all times.

In the performance of their duties, the post-holder will have access to confidential information. All such information from any source is to be regarded as strictly confidential.

Additionally the post holder should have a sound understanding of all matters pertaining to information governance, GDPR and any related policies.

Health & Safety

The post-holder will assist in promoting and maintaining their own and others’ health, safety and security as defined in any PCN policies which will include:

  • Ensuring information regarding immunity status and vaccinations status are up to date and provided.
  • Identifying the risk involved in work activities and undertaking such activities in a way that manages those risks
  • Using and having knowledge of infection control procedures particularly those relating to needle stick injuries and bodily fluids.
  • Ensuring that all accidents or incidents are reported and investigated, and follow up action taken where necessary.

  • Work effectively with individuals in other agencies to meet patients needs
  • Effectively manage own time, workload and resources.
  • Foster, maintain and grow the ethos of the PCN as an inclusive harmonious and patient centered environment.
  • Ensure any mandatory training is undertaken, kept up to date and CPD hours completed.
  • Ensure an appraisal both with a GP appraiser and the HR Director of Castleman is undertaken.

Communication

The post-holder will:

  • Recognise the importance of effective communication within the team.
  • Communicate effectively with other team members
  • Communicate effectively with patients and carers
  • Recognise people’s needs for alternative methods of communication and respond accordingly including the use of text messaging, verbal communication, video communication, interpreters etc.
