PossAbilities Shared Lives is a model of support which offers people with an assessed Social Care need e.g. Learning disability, dementia, mental ill health etc. accommodation in a family home. Shared Lives is a vibrant platform to meet new people, acquire new skills, and engage in meaningful, outcome-focused activities.

Your Purpose
You play a key role in supporting adults who need care to live fulfilling, independent lives within households in the community. Your purpose is to recruit, assess, train, and supervise Shared Lives carers, ensuring they provide safe, person‑centred support that meets regulatory standards. You will work closely with individuals using the service - listening to their wishes, matching them with the right carer, and regularly reviewing placements to make sure arrangements remain safe, supportive, and empowering.
Ultimately, you help to create stable, nurturing home environments where people can live they life they choose.
Please note: Due to the nature of this role, you will need access to your own vehicle with business insurance, as it involves regular travel.
Your Line Manager
Our Shared Lives, Team Leader.
Your Role
Your key focus will be on supporting people to achieve great things and live the life they chose, ensuring shared lives carers have the skills and knowledge to deliver person centred care, whilst providing the support they require as a carer in their own home.
You will do this through:
- Matching and placement: Matching individuals needing care with suitable Shared Lives carers and supporting these relationships to thrive.
- Building and maintaining excellent relationships: Developing and maintaining positive and effective relationships with individuals, their carers, families and circles of support. You will also be required to liaise with social work/care management teams in order to maintain relationships and ascertain the local needs for Shared Lives.
- Case management: You will be responsible for managing a caseload of arrangements, including facilitating annual service user reviews, updating support plans and risk assessments. Assisting people to be self-reliant and develop skills to reach their potential including becoming tenancy ready (if appropriate).
- Monitoring and support: Regularly visiting and monitoring placements to ensure high-quality, person-centered care, providing support to both individuals and carers
- Assessment and review: Carrying out assessments of carers, monitoring visits and annual reviews of placements in line with our procedures
- Networking and Marketing: Raising awareness of the Shared Lives scheme, promoting the service to potential carers, care managers, and engaging with local communities.
