Full job description

1. Overview of Role

The aim of this role is to ensure the accurate, timely and safe processing of all clinical correspondence and patient data within the practice. The role supports clinical decision-making by coding, triaging and managing information in line with practice protocols and relevant national and local NHS guidance and data standards.

2. Key Responsibilities

A. Workflow & Correspondence Management

· Process and triage all incoming clinical correspondence (letters, reports, lab results, emails, Docman).

· Scan and attach documents to correct patient records.

· Manage clinical mailboxes and distribute appropriately.

· Escalate urgent or high-risk information to clinicians.

B. Clinical Coding, Summarising & Data Quality

· Accurately code diagnoses, procedures, and investigations.

· Undertake detailed summarising of patient medical records, extracting and recording clinically significant information in line with practice protocols.

· Review records to ensure completeness and accuracy, identifying gaps, inconsistencies, or risks and escalating where appropriate.

· Maintain high standards of data quality aligned with NHS requirements.

C. Patient Record & List Management

· Import and manage new patient registrations.

· Assign patients to a usual GP fairly.

· Support clinical monitoring processes.

D. Reporting & Performance

· Develop and run system searches.

· Produce regular reports.

· Support KPIs and service improvement.

E. Information Technology (IT) & Systems Support

· Act as first point of contact for IT queries.

· Support staff use of systems.

· Assist with onboarding and training.

· Support IT projects.

F. Team & Improvement

· Work collaboratively with teams.

· Follow and update protocols.

· Provide team cover to reception when needed during periods of unplanned absence.

3. Accountability and Decision-Making

Accountable to the Practice Manager and Partners. Works independently within agreed protocols and ensures safe workflow processes.

4. Compliance & Governance

Maintain confidentiality, comply with data protection, safeguarding, and health & safety requirements, and ensure secure handling of patient records.

5. Person Specification

Essential

. Minimum Secondary education (Nat 5's or GSCE equivalent)

· Experience in NHS or GP administration.

· Understanding of medical terminology and record-keeping practices.

· Knowledge of clinical systems (Vision, Docman).

· Strong IT skills.

· Excellent organisation and attention to detail.

· Good communication skills.

· Ability to work independently and as part of a team.

Desirable

· Previous GP or NHS experience.

· Relevant NHS training.

· Previous experience in a summarising or clinical coding role.
