Community Nurse – Complex Care (Hertfordshire)
Location: Hertfordshire (home-based care)
Salary: £26.00 – £30.00 per hour (depending on experience)
Hours: Full or Part time
Contract Type: Ongoing placement

About the Role:
Hope Homecare Services Ltd is seeking a dedicated Community Nurse to support a young person with complex health needs in Hertfordshire.
This is a specialist one-to-one role providing holistic, nurse-led care in a home environment, working alongside a trained Support Worker and under the clinical supervision of our Lead Nurse.

The young person has a diagnosis of Early Onset Friedreich’s Ataxia, Type 1 Diabetes, Hypertrophic Cardiomyopathy, scoliosis, and other associated complex needs.
Care is delivered with compassion, clinical excellence, and a commitment to maintaining dignity and quality of life.

Key Responsibilities:

  • PEG feeding and gastrostomy care
  • Diabetes monitoring and insulin pump management (Dexcom sensor)
  • Oxygen therapy management and emergency planning
  • Mobility support including use of tracking hoists
  • Administration of medication (routine and emergency)
  • Accurate clinical documentation and safeguarding reporting
  • Working closely with family and multidisciplinary teams

Who We Are Looking For:

  • Registered Nurse (NMC PIN required) – RNC preferred but RGN or RNLD with paediatric experience welcomed
  • Confident in delivering complex paediatric care in the community
  • Excellent communication, safeguarding knowledge, and clinical record keeping skills
  • Compassionate, resilient, and committed to best practice
