Job summary

Plains View Surgery is looking for an experienced, compassionate and motivated Practice Nurse to join our friendly and supportive primary care team.

We welcome applications from candidates seeking either full-time or part-time hours. Flexible working arrangements may be considered for the right candidate.

This position requires previous experience working as a nurse within a GP practice. General practice nursing differs considerably from nursing in a hospital setting, so applicants must understand primary care systems, procedures and the responsibilities of a Practice Nurse.

Training or experience in managing long-term conditions—particularly diabetes, asthma and COPD—would be highly beneficial.

Main duties and responsibilities

The successful candidate will provide high-quality, evidence-based nursing care to our practice population. Responsibilities will include:

  • Undertaking reviews and supporting patients with long-term conditions, including diabetes, asthma and COPD
  • Running general Practice Nurse and treatment-room clinics
  • Administering adult and childhood vaccinations and immunisations, in accordance with current guidelines and individual competence
  • Undertaking cervical screening, where appropriately trained
  • Providing wound care, dressings and suture or clip removal
  • Carrying out NHS Health Checks and other health-screening activities
  • Taking clinical observations and samples, where required
  • Providing health promotion and lifestyle advice, including support with smoking cessation, diet, exercise, weight management and alcohol reduction
  • Promoting self-management and helping patients understand and manage their conditions
  • Recognising and escalating signs of deterioration or clinical concern
  • Maintaining accurate, timely and confidential electronic patient records
  • Following infection prevention and control procedures
  • Working within practice policies, clinical guidelines, Patient Group Directions and professional standards
  • Supporting the practice in meeting Quality and Outcomes Framework requirements and other primary care targets
  • Working collaboratively with GPs, nurses, healthcare assistants, pharmacists and the wider multidisciplinary team
  • Participating in clinical audits, quality improvement, training and continuing professional development

All duties must be performed within the successful candidate’s training, competence and professional scope of practice.

Essential requirements

Applicants must:

  • Be a Registered Nurse with current NMC registration
  • Have previous nursing experience within a GP practice
  • Understand the different demands and responsibilities of nursing in a primary care setting
  • Be competent in core Practice Nurse or treatment-room duties
  • Have excellent communication, clinical and organisational skills
  • Be confident working both independently and as part of a multidisciplinary team
  • Demonstrate a caring, reliable and patient-centred approach
  • Maintain accurate clinical records and patient confidentiality
  • Be committed to professional development and maintaining clinical competence

Desirable skills and experience

It would be advantageous if applicants have:

  • Recognised training or experience in diabetes management
  • Recognised training or experience in asthma management
  • Recognised training or experience in COPD management
  • Experience conducting long-term condition reviews
  • Cervical screening training
  • Childhood and adult immunisation training
  • Experience in wound care and other treatment-room procedures
  • Knowledge of QOF and primary care clinical targets
  • Experience using GP clinical systems

Pay: From £31,824.00 per year

  • On-site parking
