As a Trust we pride our-selves on our innovative and collaborative approach to providing the highest quality, person-centred care: As part of our transition to a neighbourhood model of care, we are developing integrated clinical pathways across primary, secondary care and crisis and emergency service.
Our pathways embody strong partnership working with a wide range of statutory and VCSE providers and people with lived experience.
