About the Role
The Floating Community Support Worker post has been developed to strengthen and support the existing Community Support Services and Primary Care Network (PCN) services across Hertfordshire. The role aims to help alleviate waiting times for individuals requiring support by increasing service capacity, improving access to timely interventions and ensuring that people receive appropriate support in response to their needs.
