Social Prescribing Link Worker – Warwickshire East PCN

Salary: £26,535 - £30,602 Pro rata dependent on experience

Job type: Part time. 28 hours per week. 4 days per week. 9:00am - 4:30pm.

Location: The successful candidate will be required to work in the Warwickshire East PCN. Due to the geography and nature of the role a clean UK driving license and full use of car is essential.

An exciting opportunity has arisen to join the Warwickshire East PCN as a Social Prescribing Link Worker. We are a forward thinking PCN of 5 practices and are working together to extend the social prescribing provision to our patients in the local area. We are looking for a proactive caring individual to join our small and friendly Health and Wellbeing team. Main duties of the job
Social Prescribing is a free service to help people improve patients’ general health and wellbeing in a holistic way. Through using non-medical sources of community support, Social Prescribing offers the chance to find out about activities that can improve health and wellbeing, including advice and information services, community groups, leisure activities, lunch clubs, self-help groups, specialist interest groups, sporting activities, and lots more.

As a link worker you will be working closely with the Health and Wellbeing Team, taking referrals from the surgeries, placed Based Teams, and other third-party roles.

A link worker will develop relationships in the health and social care sector, working closely with our Health and Wellbeing Coach, Care Coordinator and Clinical Pharmacists, and supporting individuals to access voluntary and community opportunities.

You will be responsible for providing non-clinical personalised support to individuals and their families, in order to empower them to take control of their own health and wellbeing, give them time and focus on ‘what matters to me’, in order to holistically improve an individual’s health and wellbeing.

Essential

· Experience of supporting people, their families and carers in a related role (including unpaid work).

· Experience of data collection and providing monitoring information.

· Demonstrable commitment to professional and personal development.

· Able to listen, empathise with people and provide person-centred support in a non- judgmental way.

· Able to get along with people from all backgrounds, respecting lifestyles and diversity.

· Able to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

· Personal accountability, emotional resilience and works well under pressure.

· Desirable: NVQ Level 3, Advanced level or equivalent qualifications or working towards.

· Desirable: Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.

· Desirable: Experience of partnership/collaborative working and of building relationships across a variety of organisations.
