General Practice Nurse – 37.5 Hours per Week
Job Summary
We are seeking a highly motivated and experienced Practice Nurse to join our clinical team in a 37.5-hour per week position. This role is ideally suited to a nurse with previous experience working in general practice, particularly in the delivery of immunisation programmes, cervical cytology, and long-term condition management.
For the right candidate, training and structured support will be provided to develop the required competencies in areas such as immunisations, cytology, and long-term condition management.
The post holder will play a key role in delivering safe, effective, and evidence-based nursing care, supporting patients with long-term conditions, delivering preventative healthcare, and contributing to national and local vaccination programmes.
A core component of this role will include undertaking home visits for housebound patients, delivering nursing care in the community where patients are unable to attend the surgery. This may include immunisations, chronic disease support, wound care, and general nursing assessments.
The role requires a clinician who is confident, compassionate, organised, and able to work both autonomously and collaboratively within the wider multidisciplinary team.
Main Duties & Responsibilities
The post holder will deliver safe and effective nursing care across a range of services, including:
- Supporting the management of long-term conditions, particularly diabetes and respiratory disease.
- Delivering cervical screening services, including cytology sampling, in line with the NHS Cervical Screening Programme.
- Delivering and supporting immunisation programmes, including:
- Routine UK childhood immunisations
- Seasonal influenza vaccinations
- Pneumococcal and shingles vaccinations
- Travel vaccinations where appropriate
- Promoting preventative healthcare and health promotion initiatives across the practice population.
- Undertaking planned home visits to housebound patients, delivering nursing care and supporting ongoing management of long-term conditions where patients are unable to attend the practice.
- Working closely with the multidisciplinary team to identify and respond to patient needs and support proactive care.
Clinical Practice
- Assess, plan, implement and evaluate care that promotes patient health and wellbeing.
- Deliver vaccinations in accordance with the UK immunisation schedule.
- Undertake cervical screening and cytology sampling in line with national guidelines.
- Identify patients at risk of developing long-term conditions and support proactive management.
- Monitor and manage patients with long-term conditions in collaboration with the multidisciplinary team.
- Provide patient education and self-management support for chronic conditions.
- Recognise patients presenting with mental health needs and refer appropriately.
- Promote and support immunisation uptake for both adult and paediatric populations.
- Carry out home visits for housebound patients as part of the planned nursing workload, providing clinical care and assessment within the patient’s home environment.
- Make appropriate referrals to other healthcare professionals and services.
This role is subject to a Disclosure and Barring Service (DBS) check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Applicants must hold current UK professional NMC registration.
