We’re looking for an experienced Practice Nurse's to support the Targeted Demand Management (TDM) service, delivering community‑based care from a mobile unit across Blackpool.

The hours will be Monday – Thursday, 08:30 – 17:00 – any availability will be considered.

Key focus:

  • Long‑term condition reviews, particularly COPD / respiratory and CVD

Requirements:

  • NMC‑registered Practice Nurse
  • Strong LTC and respiratory experience
  • Confident using EMIS Web
