Locum Medical Triager (Independent Prescriber) - Onsite

Location: Custom House Surgery, London, E16 3NA
Job Type: Part-time Locum

Hours: Monday – Friday, 1:30pm – 5:30pm
Salary: £40-50 per hour based on experience

Job Overview

We are seeking an experienced and compassionate Independent Prescriber to join our busy GP practice as a Medical Triager. This is a key clinical role, responsible for reviewing patient requests submitted through our online triage system and ensuring patients receive the right care from the right clinician at the right time.

Using your clinical expertise and judgement, you will assess patient submissions, determine the most appropriate outcome, and either manage the request remotely, arrange the appropriate appointment, or safely close the case where no appointment is required.

This is an excellent opportunity for an experienced clinician who enjoys autonomous decision-making while working as part of a supportive multidisciplinary team.

Key Responsibilities

  • Review and clinically assess patient requests submitted through our online triage platform.
  • Determine the urgency and appropriate management of each case using clinical judgement and practice protocols.
  • Provide medical advice, treatment, and prescribing where clinically appropriate.
  • Close cases that can be safely managed without the need for an appointment.
  • Book patients into the most appropriate clinician's clinic, including:
  • GP
  • Advanced Nurse Practitioner (ANP)
  • Clinical Pharmacist
  • First Contact Physiotherapist
  • Practice Nurse
  • Healthcare Assistant
  • Other members of the multidisciplinary team as appropriate.
  • Escalate urgent or complex cases promptly to the appropriate clinician.
  • Document all patient interactions accurately within the clinical record.
  • Work collaboratively with the wider practice team to ensure patients receive timely, safe, and effective care.
  • Always maintain a high standard of patient confidentiality and professional practice.

Essential Requirements

  • Registered healthcare professional (Advanced Nurse Practitioner, Paramedic or Allied Health Professional) with current registration.
  • Qualified Independent Prescriber (essential).
  • Experience working within General Practice or Primary Care.
  • Excellent clinical assessment, decision-making and patient navigation skills.
  • Strong knowledge of physiology, anatomy, pharmacology and medication management.
  • Excellent communication, organisational and IT skills.
  • Ability to work independently and confidently within agreed clinical protocols.
  • Basic Life Support (BLS) certification.

Desirable

  • Experience using online consultation or triage systems such as Accurx, eConsult, Klinik, Anima or similar.
  • Experience using EMIS Web.
  • Knowledge of NHS Primary Care pathways and multidisciplinary working.
  • Advanced Life Support qualification.
