Full job description

We are a friendly, 3-site dispensing Practice in Buckinghamshire with Surgeries in Wing, Whitchurch, and Winslow. The Practice has 6 GP Partners, together with salaried GPs and ANPs. They are supported by a full complement of Nurses, HCAs, a clinical pharmacist and provide care for 20,000 patients. We are a proactive and forward-thinking Practice dedicated to providing quality patient care. In total we are a team of more than 100 staff and we consider it important to maintain a supportive team ethos. We have a CQC rating of ‘good' across all metrics, thanks to outstanding administrative support.

The Care Coordinator will play an important role within 3W Health to proactively coordinate care activities and provide administrative support for the delivery of patient care.

Hours of Work -

Monday 08:00 to 18:00

Tuesday - Off

Wednesday 08:00 to 18:00

Thursday 08:00 to 18:00

Friday 08:00 to 18:00

Aim of the post:

  • To provide comprehensive administrative support to the Practice
  • To support the Practice to deliver outcomes defined within the enhanced service and quality improvement schemes which they sign up to
  • To help coordinate care needs across the Practice
  • To utilise Population Reporting within the clinical systems to help identify target patient populations
  • To ensure colleagues are made aware of any planned patient communications, service changes and offering guidance and help as required

Main responsibilities:

  • To record patient interventions on relevant electronic database systems (e.g. EMIS) and contribute to report generation, analysis and production
  • To be customer (patient, carer, GP) focused when representing the service
  • To work collaboratively with other teams and services to maintain an effective and efficient service
  • To plan / organise work using own initiative, whilst being able to work as a valuable member of a team
  • To undertake general office duties to support the role
  • To ensure all electronic records are updated and complete within the agreed timescales
  • To use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers and families and colleagues
  • To participate in the review and appraisal process
  • To carry out any other reasonable duties as requested by a manager to ensure quality of service
  • To participate in any relevant training / courses /conferences and complete mandatory training
  • To use clinical systems for record keeping, audit and evaluation
  • To develop and implement data collection systems that will provide accurate and timely data.
  • To always maintain confidentiality

Experience

Essential

  • Good knowledge of IT systems and packages
  • Experience of data collection and reporting
  • Experience of working within a patient-facing role
  • Experience of collaborative working and building relationships

Desirable

  • Previous experience in a medical setting
  • Experience of working within a patient-facing role
  • NVQ level 3 or equivalent in a health and social care role or business administration.

Skills

· Demonstrable commitment to professional and personal development

  • Ability to organise and prioritise workload effectively
  • Excellent administration skills to include record keeping, communication (written, verbal & listening skills)
  • Ability to work autonomously as well as within a team
  • · Computer literate – excellent IT skills in Microsoft Outlook, Word, Excel & PowerPoint and the internet and intranet
