Overview:
Akerman Medical Practice is seeking an enthusiastic, patient-centred and committed Salaried GP to join our multidisciplinary team. The post has been designed to provide a flexible portfolio-style GP role, combining continuity of care for care home residents with alternate Saturday clinical sessions.
The position may be particularly suitable for a GP who has existing weekday employment or portfolio commitments and wishes to supplement their current sessions with a regular, structured role incorporating weekend working and flexible care home responsibilities.
The post-holder will be responsible for delivering safe, effective and evidence-based primary medical care while maintaining the highest professional and clinical standards.
We are looking to develop a stable, long-term working relationship with the successful applicant. This position is ideal for motivated GPs seeking a rewarding role that combines clinical excellence with community impact.
Duties:
- Conduct thorough medical assessments and examinations for residents in care homes, ensuring personalized treatment plans
- Manage acute and chronic health conditions, including medication administration and treatment adjustments
- Collaborate with multidisciplinary teams to coordinate patient care, including nursing staff, specialists, and social workers
- Perform routine health checks, screenings, and health promotion activities tailored to senior and hospice care populations
- Document all clinical encounters accurately in electronic health records while maintaining confidentiality
- Provide weekend coverage by delivering prompt diagnoses and interventions for urgent medical issues
- Support clinical research initiatives or public health programs aimed at improving elderly care standards when applicable
Working Pattern:
The post consists of two care home sessions together with alternate Saturday clinics.
The detailed timetable and working arrangements will be agreed with the successful candidate and the Lead GP.
Care Home Session 1 – Ward Round & Associated Administration
The post-holder will undertake a regular planned care home session incorporating:
- Scheduled care home ward rounds.
- Clinical assessment and review of residents.
- Assessment of acute and ongoing clinical concerns.
- Management of chronic and complex health conditions.
- Frailty and elderly care management.
- Medication reviews where clinically appropriate.
- Review of investigation results and correspondence.
- Referrals to secondary care and community services where appropriate.
- Advance care planning and anticipatory care where clinically appropriate.
- Liaison with care home staff, relatives and other healthcare professionals.
- Completion of associated clinical documentation and administrative work.
- Accurate and contemporaneous documentation on EMIS Web.
Care Home Session 2 – Flexible / Ad Hoc Clinical Support
The second care home session will be delivered flexibly across the working week to provide clinical support for issues arising between planned ward rounds.
Responsibilities may include:
- Responding to clinical queries from care home staff.
- Reviewing changes in residents' clinical condition.
- Providing appropriate clinical advice and management plans.
- Responding to medication-related queries.
- Reviewing results and correspondence.
- Arranging appropriate investigations or referrals.
- Liaising with community, secondary care and multidisciplinary services.
- Reviewing residents remotely where clinically appropriate.
- Addressing other ad hoc clinical concerns requiring GP input.
The precise arrangements for availability, response expectations and workload will be agreed with the Lead GP to ensure the arrangement is clinically safe and manageable.
Saturday Clinical Sessions
Alternate Saturday clinics are an integral part of this salaried GP role.
The Saturday working pattern will alternate between:
Remote Saturday Session
The GP will undertake the agreed clinical session remotely, including telephone or other appropriate remote consultations.
Practice-Based Saturday Session
The following Saturday clinic will be undertaken from Akerman Medical Practice.
The clinical list will contain an appropriate combination of telephone and face-to-face consultations. The practice-based Saturday session should therefore not be interpreted as an exclusively face-to-face clinic. The alternating remote/practice-based arrangement will continue according to the agreed rota.
Skills:
- Proven experience in patient care with a focus on senior or hospice populations
- Strong knowledge of anatomy, physiology, and pharmacology relevant to elderly and vulnerable groups
- Experience with nursing home environments or home care services is highly desirable
- Familiarity with medical imaging techniques and their application in diagnosing complex conditions
- Ability to perform medication administration safely and effectively within scope of practice
- Knowledge of public health principles to promote community well-being among elderly populations
Person Specification:
Essential Qualifications & Registration
The successful applicant must:
- Be a qualified General Practitioner.
- Hold full GMC registration with a licence to practise.
- Be included on the GMC GP Register.
- Be included on the appropriate NHS England Medical Performers List or meet the necessary eligibility requirements.
- Hold MRCGP or an equivalent recognised qualification.
- Maintain appropriate medical indemnity.
- Have an up-to-date NHS GP appraisal and revalidation status.
- Hold an Enhanced DBS certificate or be willing to undertake the appropriate check.
- Have the right to work in the United Kingdom.
Remuneration & Benefits:
Salary: £11,000 pro rata per session, per annum.
