About the role:

The job sits within Harbour Housing, a Homelessness Charity whose primary objective is to relieve the need, hardship, and distress of vulnerable homeless people. .

Job Description

To work as part of a team, which promotes social inclusion and access for service users in recovery from mental health difficulties, substance misuse issues and experience of homelessness. The teams focus is on the individual needs and wishes of service users, working across boundaries of care in enabling and empowering individuals to access opportunities and promote their recovery, with a specialist emphasis on supporting people to resolve housing-related issues such as sustaining a tenancy and obtaining long-term supported social or private housing.

The Support, Time and Recovery worker will be required to provide support and give time to an allocated group of service users with complex needs in order to promote their recovery and maintain them in their community environment.

Job Purpose

To support service users to manage a variety of issues that affect their ability to live their lives, such as:

  • Homelessness
  • Substance Misuse (Alcohol and Drugs)
  • Social Exclusion
  • Challenging Behaviour
  • Unemployment
  • Basic Living Skills
  • Mental Health
  • Domestic Abuse and Sexual Violence

This will be achieved by:

  • Identifying and understanding what services users need and building relationships
  • Planning a bespoke programme of support with a focus on housing-related support provision aimed at securing long-term accommodation solutions
  • Working with families and friends (Co-Production)
  • ·Referring and signposting service users to other agencies that can help
  • Recording information and updating records diligently and appropriately

Key Accountabilities:

  • To provide individual and/or group interventions to service users in accordance with agreed support plans both within Harbour Housing schemes and in individual’s own homes/local community.
  • To support and monitor service users and to share as appropriate with relevant members of the multidisciplinary team and other external services.
  • To seek advice and guidance from Senior Staff as appropriate.
  • To positively promote independent living of service users both within Harbour Housing Schemes and in the community.
  • To develop a rapport with service users based within appropriate and transparent boundaries
  • To provide regular and therapeutic activities to service users in devleloping and managing their independence.
  • To be aware of the escalation process to highlight any service user potential risk in a timely manner to contribute to ongoing risk management and reduction.
  • To support service user to gain access to resources and services including benefits, welfare rights and other statutory and therapeutic services.
  • To provide information on health promotion.
  • To work with service users on move-on strategies to support and enable to them to secure long-term accommodation that is both appropriate and attainable.
  • Act as an advocate for vulnerable individuals in receipt of care and support, working with a person-centred approach and ensuring that the individual is placed at the centre of all care and support delivery.
  • To conform to all Policies and Procedures laid down by the Organisation detailed within the Company’s employee handbook and Health & Safety handbook in respect of carrying out these duties and in other administrative aspects of the business, as relevant.
  • To participate as directed by the Training and HR Team in regular In-service training programmes.
  • Develop, monitor and maintain accurate administration records relating to individuals’ support in order to form an accurate picture of progress towards achievement of outcomes and improvements in individual health and wellbeing.
  • To participate in Staff, Team and Quality Management Review Meetings as directed by Team Leaders/Managers and to participate actively in reviews of service users’ documentation as required.
  • To report back to the Housing Serviced Team Lead/Safeguarding Team/Harm Reduction Team on any aspect of service user support that they feel warrants investigation or urgent action.
  • Build and maintain knowledge of local services and make links with relevant organisations to develop the care and support services offered and to promote business aims.
  • Ensure the management and maintenance of relationships with internal and external stakeholders, identifying any shortfalls in service delivery and ensuring they are dealt with appropriately.
  • Contribute, as appropriate, to special programmes and projects in support of the organisation’s objectives.

The post is subject to an enhanced DBS check and references.

Due to the nature of the role, the sucessful candidate must hold a full UK driving licence
