Social Prescribing Link Worker and Health & Wellbeing Worker (blended role)
Working pattern: Two days providing social prescribing support, primarily working from home, and three days delivering face-to-face health and wellbeing support
About The Role
We are looking for a compassionate, motivated and person-centred individual to join our team in a blended role combining social prescribing with practical health and wellbeing support.
This is one full-time position, and the two elements of the role will not be separated. The successful candidate will need to feel confident working independently from home as well as providing face-to-face support within GP practices and community settings.
The postholder will work alongside individuals to understand what matters to them, identify barriers affecting their health and wellbeing, and help them access the right support. This may include connecting people with community groups, voluntary organisations and specialist services, as well as providing more focused one-to-one support to help people make sustainable lifestyle changes.
The role requires someone who can build trusted relationships, work creatively and recognise that people’s health and wellbeing are often affected by a range of social, emotional, practical and environmental factors.
Key Responsibilites
Social prescribing
- Receive and manage referrals from GP practices and other agreed professionals within Truro PCN.
- Provide person-centred conversations that focus on what matters to the individual.
- Work with people to identify their strengths, needs, goals and priorities.
- Develop appropriate and achievable support plans with individuals.
- Connect people with community groups, voluntary organisations, statutory services and other local support.
- Support individuals who may be experiencing loneliness, social isolation, financial difficulties, housing concerns, low confidence, poor mental wellbeing or other challenges affecting their quality of life.
- Maintain regular contact with individuals and review progress against agreed goals.
- Build and maintain knowledge of local services, community activities and available sources of support.
- Develop positive working relationships with GP practices, community organisations and partner agencies.
- Promote social prescribing and help professionals understand the support available through the service.
Health and Wellbeing Support
- Provide face-to-face, one-to-one health and wellbeing support within GP practices and community settings.
- Use coaching-style conversations to help individuals identify their own goals and solutions.
- Support people to make realistic and sustainable changes to improve their physical and emotional wellbeing.
- Work with individuals around areas such as healthy eating, physical activity, weight management, confidence, motivation, social connection and emotional wellbeing.
- Help people recognise and overcome barriers that may prevent them from making positive changes.
- Use motivational interviewing and other person-centred approaches where appropriate.
- Agree practical action plans and regularly review progress with the individual.
- Encourage people to build confidence, independence and the skills needed to manage their own health and wellbeing.
- Signpost or refer individuals to specialist services when their needs fall outside the scope of the role.
- Deliver support in a way that is inclusive, non-judgemental and responsive to individual needs.
General Responsibilities
- Manage a varied caseload across both elements of the role.
- Maintain accurate, timely and confidential case notes on all relevant recording systems.
- Ensure outcomes, activities and participant progress are recorded in line with contractual and organisational requirements.
- Work within agreed boundaries and recognise when concerns need to be escalated.
- Follow safeguarding procedures and report concerns appropriately.
- Attend regular supervision, team meetings, training and development opportunities.
- Work collaboratively with colleagues across CHAOS Group, Truro PCN and wider partner organisations.
- Contribute to service monitoring, evaluation and continuous improvement.
- Gather feedback and case studies, with appropriate consent, to demonstrate the difference the service is making.
- Promote equality, diversity and inclusion in all areas of work.
- Follow all CHAOS Group policies and procedures, including those relating to confidentiality, data protection, lone working, health and safety and safeguarding.
- Undertake any other reasonable duties appropriate to the level and purpose of the post.
Person Specification
Essential
- Experience of supporting people with health, wellbeing, social or emotional needs.
- A strong understanding of person-centred working and the importance of focusing on what matters to the individual.
- Excellent communication and active listening skills.
- The ability to build positive and trusting relationships with people from a wide range of backgrounds.
- A compassionate, respectful and non-judgemental approach.
- The ability to motivate and empower people without creating dependency. Confidence in working both independently and as part of a wider multidisciplinary team. The ability to manage a varied caseload, organise workload and meet deadlines.
- Good IT skills and the ability to maintain accurate and confidential records.
- An understanding of professional boundaries, confidentiality and safeguarding.
- A willingness and ability to work flexibly across home, GP practice and community-based settings.
- A commitment to equality, diversity and inclusive practice.
- Access to appropriate transport to travel between locations as required.
Desirable
- Previous experience in social prescribing, health coaching, community development, mental health, primary care or a related role.
- A relevant qualification in health coaching, social prescribing, counselling, health and social care or community work.
- Experience of using motivational interviewing or coaching approaches.
- Knowledge of local community services and voluntary-sector organisations across Truro and the surrounding area.
- Experience of working within a Primary Care Network or alongside GP practices.
- Experience of using clinical or case-management recording systems.
- Training or experience in safeguarding, mental health awareness and behaviour change.

Additional Requirements
The successful candidate will be required to complete an appropriate Disclosure and Barring Service check. The role may involve travel across the Truro PCN area and occasional attendance at meetings or events outside the usual working pattern.
