Background: IMPAKT Housing & Support has been providing homes and support to people with complex needs since 1991. We are a Registered Social Landlord, registered charity, and Psychologically Informed Environment (PIE) provider, and we take all three seriously. We directly house over 400 people across Bedfordshire, Cambridgeshire, and Northamptonshire who have experienced homelessness, domestic abuse, or trauma, and we work hard every day to make sure that a home with IMPAKT is the stable foundation from which people can rebuild their lives.

Beyond our directly managed provision, we deliver domestic abuse outreach services across the East of England, operate a community food larder, and run social enterprise retail stores. We are also developing a growing partnership housing model, working alongside specialist third-sector organisations across the Southwest, Kent, and North and South Yorkshire to extend our reach.

Responsible to: Head of Housing North

Job Purpose: In recent years, we have seen an increase in numbers of people residing in our supported accommodation projects experiencing poor mental health. The purpose of the role is to improve the quality of care we are able to deliver and to generate better outcomes for these residents. Using your excellent clinical knowledge, in conjunction with the Mental Wellness Team Leader, you will liaise with residents, housing coaches & healthcare professionals to establish an effective interdisciplinary care plan, based on the individuals needs and wishes. To be the ‘go-to’ person for keeping the individual’s service provision on-track, monitoring and evaluating progress to ensure that best outcomes are achieved.

Main Duties and Responsibilities:

• To be a primary point of contact for referrals where concerns have been identified over an individual’s mental health
• To manage a caseload of residents, listening to identify the needs and wishes of the individual, building trusting relationships and supporting them to develop a personalised care and support plan
• Work in partnership with the housing coaches to connect people to appropriate community and emergency support services
• Keep housing coaches informed of progress or decline in mental health and any changes to risk level
• Support residents to attend appointments – including at points of crisis, making applications that may improve mental wellbeing where appropriate
• Monitor and review progress, amending the support plan where necessary and keeping accurate records
• Work with skills coaches to help residents in your caseload to access therapeutic activities and actively contribute to the development of wellbeing activities
• To travel to projects across the county to meet with residents as required.

Additional Duties and Responsibilities:

• Be flexible to the changing needs of the organisation and its service users.
• Abide by the organisations staff working policies and procedures.
• Promote the aims, purposes, mission and services delivered by the organisation.
• In addition to the duties set out in this job description the post holder may at times be required to undertake additional or other duties to meet the needs of the organisation.

This in an in-person role, working Monday - Friday, 9AM-5PM.

Locations: IMPAKT Housing & Support has project locations throughout Bedfordshire, however this post holder will be predominantly based in Bedford, The Foyer.
