Homes in Somerset is looking for a compassionate and proactive Case Manager to join our Supported Housing team on a 12-month fixed-term contract. This is a rewarding opportunity to support customers to sustain their tenancies, achieve personal goals, and improve their wellbeing through tailored, person-centred support.

What You'll Be Doing

  • Managing a caseload of customers living in supported housing.
  • Developing and reviewing personalised support plans.
  • Helping customers build independence and maintain successful tenancies.
  • Working closely with partner agencies and support services.
  • Identifying and managing risks, including safeguarding concerns.
  • Maintaining accurate records and support documentation.
  • Supporting customers to access services relating to health, employment, finances and wellbeing.
  • Delivering a high-quality, customer-focused service that achieves positive outcomes.

You will be

  • Experienced in housing, support work, social care, case management or a similar role.
  • An excellent communicator with the ability to build trust and positive relationships.
  • Organised and able to manage a varied caseload.
  • Resilient, empathetic and solution-focused.
  • Confident working independently and as part of a team.
  • Committed to delivering great outcomes for customers.
