Company Overview

Impression Health and Support UK Ltd is a North West organisation specialising in neurological rehabilitation and long term care.

We are dedicated to delivering effective services, bespoke packages, and collaborative solutions with multidisciplinary teams to meet the unique needs of our patients and support their journey towards independence.
Summary

We have an exciting post opened for a Admission & Discharge Co-ordinator within Neuro Services.

We see this role as vital in ensuring smooth patient admissions, stay and discharge. The role will require you to contributed and become an integral part of our in house mdt.

We are seeking somebody with a passion to be part of the organisations expansion plans as we develop new and existing services.

Currently our services are operating at 90% occupancy.
Responsibilities

  • Coordinate patient admissions and discharges efficiently, ensuring all documentation is accurate and complete to a high standard.
  • Liaise with multidisciplinary teams including neuro rehab consultants, psychologists, physiotherapists, speech therapists, language therapists, and occupational therapists.
  • Maintain detailed records of patient progress and service delivery.
  • Schedule appointments and manage communication between patients and healthcare professionals where required.
  • Ensure compliance with healthcare regulations and company policies.
  • Support the development of individualised care plans in collaboration with the team prior to admission.
  • Assist in administrative tasks related to patient care coordination.
  • Contribute to continuous improvement of service processes.
  • Time efficient and ability to prioritise.
  • Qualifications
  • Nurse background or clinician background required.
  • Assessement reports and discharge summary documents are required to high standard.
  • Excellent organizational and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Strong attention to detail and accuracy.
  • Proficiency in office software such as MS Office.
  • Knowledge of healthcare regulations is a plus.
  • Prior experience in neurological or rehabilitation services is a necessity.
  • Ability to handle sensitive information with confidentiality.
  • Prior working or knowledge of local commissioning services, ICBs desirable.
