Full job description
Seeking experienced Healthcare assistants to work full time/part time; Experience of using Emis web is desirable, across 3 sites as part of JS Medical ( Park Lane N17 0JP , Philip Lane N15 4JR and Westbury N22 6RS) in Haringey.
Expected competencies to include:
- New patient health checks and other health screening
- Urinalysis and preparation of specimens
- Blood tests and phlebotomy
- Long term condition reviews where appropriate
- Vaccinations including flu/pneumococcal vaccines
- Patient education and promotion of health and well-being
- Gathering lifestyle data from patients
- Infection control and equipment management
- Vaccination management and clinical stock control
Confidentiality:
The post-holder will have access to sensitive patient and practice information and must uphold the highest standards of confidentiality. All patient information must be treated with respect, adhering to the practice’s policies and procedures on confidentiality and data protection.
Health & Safety:
The post-holder will promote health and safety within the practice, including:
- Adhering to health and safety policies and procedures.
- Identifying potential risks and taking appropriate action.
- Following infection control protocols and maintaining a safe working environment.
Communication:
The Care Coordinator must effectively communicate with:
- Team members to ensure coordinated care delivery.
- Patients and their families, recognising diverse communication needs.
- Other members of the public when required to support patient care.
Quality Assurance:
The post-holder will contribute to maintaining and improving quality standards by:
- Identifying and reporting quality issues or risks.
- Participating in quality assurance initiatives and audits.
- Reflecting on personal performance and seeking opportunities for professional development.
- Utilising QOF, local target and KPI data to inform practice improvements and ensure high-quality patient care.
Personal/Professional Development:
The post-holder is expected to participate in ongoing training and development activities, including:
- Engaging in annual performance reviews and maintaining a record of professional growth.
- Staying informed about current best practices in patient care and quality improvement.
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
