Job Overview
We are seeking a dedicated Hospital Liaison Worker to serve as a vital link between healthcare providers, patients, and community organisations. This role involves providing practical and emotional support to individuals with brain injury and their families within various hospital settings. The role focuses on ensuring smooth transitions from hospital to community services, promoting recovery, reducing readmissions, and improving long-term outcomes.
The post-holder will act as a key point of contact between hospital teams, patients, families, and community services, ensuring coordinated care and access to appropriate support.
Hours: 21 hours per-week
Pay: £24,500 per annum, gross pay (pro rota)
Responsible to: Service Manager
Responsible for: Lived Experience Volunteers
Key Responsibilities
- Provide information, advice, and guidance to patients with brain injury and their families during hospital stays and discharge planning.
- Support safe and effective transitions from hospital to home or community-based services.
- Liaise closely with multidisciplinary hospital teams (e.g. consultants, nurses, therapists, discharge coordinators).
- Build relationships with external agencies to support continuity of care, including social care, GPs, and voluntary sector services.
- Advocate on behalf of patients and families to ensure their needs are understood and met.
- Identify early support needs, including emotional, practical, and social care requirements.
- Assist patients and families in accessing benefits, funding, and relevant community services.
- Provide signposting and referrals to appropriate rehabilitation, support groups, and community resources.
- Maintain accurate records of interactions, interventions, and outcomes.
- Monitor and report on service effectiveness, highlighting gaps or emerging needs.
- Contribute to the ongoing development of the liaison service within hospital settings.
Skills
- Knowledge of the causes of brain injury and its effects on individuals and/or other long-term conditions (Essential).
- Basic counseling/advocacy skills (Desirable).
- Experience of working in social care environments with complex behaviours (Essential).
- Knowledge of hospital discharge processes or care pathways (Essential).
- Experience of working with a wide range of people with complex behaviours and operating in a variety of environments (Essential).
- Minimum Level 2 qualifications in English and Mathematics (Desirable).
- Good IT skills – including use of Word, Excel, Access, Email & Internet (Essential).
- Willingness to undertake training to support role (Essential).
- Experience working within hospital environments (Desirable).
- Knowledge of the Welfare Benefits system (Desirable).
- Personable, open and non-judgmental (Essential).
- Able to work well within the whole staff team (Essential).
- Punctual and able to work on own initiative, planning and prioritising workload to meet agreed targets (Essential).
- Good written and verbal communication skills (Essential).
- Ability to create and maintain client records (Essential).
- Ability to advocate on behalf of service users and carers (Desirable).
