Project Description
Neath Port Talbot Prevention and Wellbeing Support provides generic floating support to the residents of Neath Port Talbot. Addressing housing, financial and personal difficulties, building the service users skills and confidence, improving their ability to maintain their accommodation, increasing their independence to achieve agreed goals.
Purpose of Role
The Intensive Housing Worker will provide high-quality, person-centred, trauma-informed support to individuals with complex needs who are homeless or at risk of homelessness, helping them achieve and maintain stable, independent accommodation.
Support will be delivered across both community and residential settings, requiring the post holder to work flexibly to meet the needs of individuals in their own homes, temporary accommodation, and wider community environments.
Working within a housing-led and solution-focused approach, the post holder will coordinate a varied caseload, undertake assessments, develop support and risk management plans, and work closely with partner agencies to ensure clients can access the services and opportunities they need.
The successful candidate will demonstrate professionalism, empathy, and a friendly, approachable manner, enabling them to build trusting relationships with clients, colleagues, landlords, and external stakeholders.
They will possess moderate IT skills and be confident using case management systems, Microsoft Office applications, email, and digital record-keeping to maintain accurate documentation and monitor outcomes.
As part of a supportive team of approximately 20 staff, the post holder will be expected to work collaboratively, contribute positively to team objectives, communicate effectively, and provide cover for colleagues when required. A commitment to equality, safeguarding, positive risk-taking, harm reduction, and empowering individuals to develop the skills and confidence needed for independent living is essential to this role.
This role is subject to an Enhanced DBS check
Please note: Driving Licence and access to own vehicle is essential for this role.
