Introduction

This is an exciting opportunity for someone who is committed highly motivated, and performance orientated, to join an established team. We are looking for a passionate and dedicated individual to be part of Lewisham’s model of Integrated Health and Social Care, with multi-disciplinary teams including the voluntary sector, to promote independence and personalisation The main purpose of this job is to work as a care management officer to assess the needs of vulnerable adults and their carer’s.

What’s the role?

As a Case Management Officer you will be required to assess the needs of vulnerable adults and their careers, and to support them to arrange services to meet the needs. In addition to this you will be assessing and reviewing Adult social care plans, which is a key component of this role as well as recording accurately on case files.

The main purpose of this job is to work as a care management officer to assess the needs of vulnerable adults and their carer’s.

This role is for 1 part-time (17.5 hours per week) Case management officer.

Location

The job is primarily based in Laurence House, but we operate a hybrid approach where you will be required to work both in the office and from home on a regular basis. You will also be required to visit service users in their homes.

Salary

£41,739 to £43,014 per annum (SO1) pro rata - 17.5 hours

N.B: All appointments will be offered at the minimum spinal column point (SCP) for the grade of the role. The only exception to this will be when matching current salary (up to the maximum of the scale), which is subject to evidential proof.
