Full job description

Are you passionate about helping people to achieve their goals and to lead healthier, happier lives?

We are seeking to recruit a Health and Wellbeing Coach to work across two different surgeries: Gratton Surgery and Broughton Surgery (Broughton is the branch surgery of Stockbridge Surgery). The successful candidate will review referrals from the GP team, and book individual appointments with patients. The focus is supporting patients to improve their physical, mental and emotional wellbeing through personalised coaching and behaviour change support. This starts with helping patients identify goals and includes support to build healthy habits, improve self-management of low-level mental health conditions and increase confidence in managing their own health & wellbeing.

We welcome applications from candidates with a background in coaching and a relevant qualification, ideally the Personalised Care Institute (PCI) accredited Health and Wellbeing Coaching programme. You should be confident in working unsupervised and in knowing the limits of your own competency.

Interview Date: Thursday 19 November in the morning only

Pay: £15.00-£16.50 per hour

Expected hours: 15.5 per week
