Role: ICU Resident Doctor / Intensive Care Fellow

Locum shifts available - shifts from 8 hours up to 25 hours.

JOB SUMMARY

· To provide support and work closely with all medical, nursing and other appropriate staff to facilitate the excellent standard of care given to critically sick patients at The London Clinic.

· To review all patients on the critical care units at least three times per day and to undertake rounds with the consultants.

· To provide emergency care for severely deteriorating patients likely to require high dependency critical care, pending the arrival / input of the designated consultant and / or the identification and involvement of the consultant anaesthetist / intensivist.

· To keep all consultants informed in changes in critical patient’s condition and to agree and action any new treatment.

· To ensure all records are efficiently and accurately set up maintained and utilised, including manual and computerised records for reporting and audit procedures

DUTIES & AREAS OF RESPONSIBILITY

This job description is a general guide to the scope of the post but may change in line with the needs of the department or at the request of the Manager. The post-holder may be asked to undertake any other project or duties commensurate with the grade of the post and as agreed with the Manager.

· To carry and respond to the bleep at all times.

· To manage each patient in conjunction with a named consultant, and to implement the guidelines laid down by the ICU Consultant advisor to ensure that all critically ill patients receive the highest standard of medical care.

· To provide support to the clinical assistants and ward staff on immediate care to ensure that very sick patients have appropriate assessment and treatment and in the absence of the admitting consultant, to transfer to ICU and arrange immediate Consultant Intensivist cover

· To be an integral part of the Outreach Team and to provide medical input to ‘at risk’ patients.

· To respond to cardiac arrest calls as second medical attender. (The RMO carries the crash bleep and is the first in attendance).

· To escort and monitor critical care patients for investigations and interventions (but not operative surgery except in extreme situations and where appropriate clinical skills allow).

· To assist Consultant Anaesthetists with critical care patients for anaesthetics, when undergoing surgery if required to do so, and if clinical skills allow.

· To ensure that each patient contact is documented (and timed and dated) within the patient’s medical record including any ward / unit patient for whom care or advice has been provided.

· To ensure appropriate external transport for critically ill patients.

· To ensure that face to face hand over occurs between the outgoing and incoming rostered staff member/ ICU fellow. To ensure that all other medical staff are updated as to the condition of any of their patients who may have been seen in their absence.

· To ensure that all appropriate medical staff have adequate updates in the care of critically ill patients. This will include teaching nurses, RMOs and clinical assistants and any other groups, when required.

· To ensure that personal clinical knowledge is up to date and all care given is evidence based.

· To provide emergency cover / counselling for Sharps Injuries at 5 Devonshire Place.

· To take part in and / or initiate specific clinical audits when required to do so. To identify and initiate in conjunction with appropriate Consultants, relevant and opportunistic clinical audits if appropriate.

· To participate in unit governance meetings and preparation for these (e.g. mortality & morbidity reviews) and updating local clinical guidelines.

· To attend all RMO / Clinical Assistant medical meetings at either hospital to ensure general hospital awareness is maintained.

· To undertake any other task commensurate with grade requested by Consultants or senior management at either hospital.

· To uphold all Policies / Procedures / Guidelines of the hospital and individual hospital departments.

· To ensure ALS, PILS and all mandatory training is kept up to date.

CRITERIA

  • GMC registration
  • Minimum 6 months' UK experience in ICU
  • Minimum of three years post qualification medical/surgical.
  • Minimum of twelve months intensive care experience
  • Minimum six months anaesthetic experience or airway logbook and competencies if non-anaesthetic training.
  • Valid Advanced Life Support (ALS)
