Clinical Rehabilitation Coordinator
Full time, part time or flexible working considered

We are an expanding specialist community rehabilitation service supporting adults with complex neurological, cognitive and physical rehabilitation needs within their homes and local communities. We work collaboratively with clients, families, case managers and multidisciplinary professionals to deliver high-quality, person-centred rehabilitation support tailored to individual goals and outcomes.
Due to continued growth and service development, we are looking to recruit a Clinical Rehabilitation Coordinator to support the coordination, clinical oversight and ongoing development of our rehabilitation services.
The role
This is a varied and rewarding role combining clinical coordination, staff support, service development and quality oversight. The successful candidate will play a central role in supporting both clients and rehabilitation staff, helping to ensure services remain safe, responsive, clinically effective and person centred.
The role is primarily home-based, with flexibility to attend client visits, assessments, multidisciplinary meetings and staff support sessions where required.
Key responsibilities include:
- Coordinating and overseeing community rehabilitation support packages
- Completing initial assessments and contributing to rehabilitation planning
- Developing and reviewing rehabilitation plans, support plans and risk assessments
- Supporting the onboarding, induction and supervision of rehabilitation assistants
- Providing ongoing clinical guidance, support and problem-solving for rehabilitation staff
- Monitoring clinical quality, documentation and standards across rehabilitation packages
- Supporting recruitment, training and staff development processes
- Liaising with case managers, therapists, families and external professionals
- Attending and contributing to multidisciplinary team meetings
- Supporting clinical governance, reflective practice and quality improvement initiatives
- Contributing to the development of policies, procedures and clinical resources
- Assisting with service development projects as the organisation continues to expand
- Maintaining accurate, professional and timely clinical documentation
Additional opportunities
For candidates from an Allied Health Professional background, there may also be opportunities to undertake additional independent clinical work alongside the coordinator role, depending on experience and areas of expertise. This may include involvement in private multidisciplinary rehabilitation input, specialist assessments, therapy intervention, joint working alongside case managers and treating professionals, and participation in complex community rehabilitation programmes.
The role offers the opportunity to work closely within private and medico-legal multidisciplinary teams, contributing to holistic rehabilitation planning for individuals with complex neurological and catastrophic injuries. There may also be opportunities to contribute to service development projects, staff training, clinical governance initiatives and the ongoing development of specialist rehabilitation pathways as the service continues to grow.
About you
You will be organised, proactive and clinically confident, with the ability to balance autonomy, coordination and staff support within a community rehabilitation setting.
Essential requirements:
- Registration with the HCPC
- Experience working with individuals with complex neurological and/or catastrophic injuries
- Experience completing assessments, rehabilitation plans and risk assessments
- Strong organisational and communication skills
- Ability to work independently and manage competing priorities
- Confident working collaboratively with multidisciplinary professionals
Desirable:
- Experience supervising, mentoring or supporting rehabilitation assistants or support workers
- Experience working within community rehabilitation, case management or medico-legal settings
- Interest in service development, quality improvement and staff training
- Experience contributing to policy development or clinical governance processes
- Experience working within private rehabilitation or community-based MDT environments
- Full UK driving licence and willingness to travel within the community as required
