Overview
Join our team on a casual basis to support the delivery of a high-quality, safe, and efficient Primary Care–led anticoagulation (INR) monitoring service for patients prescribed warfarin. The post-holder will carry out point-of-care INR testing, support dose management under protocol, and work collaboratively with multidisciplinary teams across Brighton & Hove Federation. Clinics will take place at GP practices in the West, Centre and East of the city. There is also the opportunity to provide domiciliary visits to housebound patients.
Main duties and responsibilities
Clinical Duties
· Perform point-of-care (PoC) INR testing using approved devices in domiciliary, community, or GP practice settings
· Ensure accurate recording of INR results and relevant clinical information in INRStar and subsequently in the Patient Medical Record (PMR)
· Support protocol-driven anticoagulation monitoring processes using INRStar within agreed competency limits and escalation pathways
· Recognise and escalate out-of-range INR results, red flag symptoms, or clinical concerns to the supervising clinician in accordance with service protocols
· Obtain and document relevant information affecting anticoagulation control including adherence, diet, recent illness, medication changes (acute and repeat) and potential drug interactions
· Provide basic education on warfarin use, including:
o Adherence
o Diet and lifestyle considerations
o Recognition of bleeding risks and when to seek help
· Work within agreed competency frameworks, SOPs, escalation pathways and service protocols at all times
Domiciliary or community visiting
· Undertake domiciliary visits for housebound or vulnerable patients requiring anticoagulation monitoring, in accordance with service protocols, risk assessments and lone working procedures
· Perform PoC INR testing safely within patient’s homes or other community settings using approved equipment and processes
· Support protocol-led anticoagulation monitoring activities during domiciliary visits
· Assess the suitability and safety of the environment for clinical activity and work in accordance with infection prevention, safeguarding, moving and handling, and health and safety policies
· Ensure the safe handling, transport, storage, cleaning, and maintenance of point-of-care testing equipment, consumables, and clinical supplies used during community visits
· Maintain confidentiality, dignity, professionalism, and a patient-centred approach whilst delivering care in patients’ homes
Patient Care
· Book, coordinate and manage patient appointments for anticoagulation and INR monitoring clinics
· Ensure patients understand testing procedures, authorised dosing instructions, follow-up arrangements, and when to seek urgent medical advice
· Provide patient centred support and education to promote safe anticoagulation monitoring and treatment adherence
· Maintain a patient-centred approach at all times, ensuring dignity, respect, confidentiality, and equal access to the service
· Support housebound, vulnerable or high-risk patients where service delivery includes outreach or domiciliary visiting
· Assist patients with accessing appropriate follow up care and onward referral where required
Medicines Management
· Support the safe monitoring and management of anticoagulation therapy in collaboration with service pharmacists, prescribers and the wider multidisciplinary team
· Identify and escalate potential drug interactions or other factors that may impact anticoagulation therapy
· Assist in medication review process as directed by supervising pharmacists and prescribers within agreed competency limits and escalation pathways
· Promote safe anticoagulation monitoring and medicines management practices
Data Management & Administration
· Record INR results accurately in INRStar in line with Point of Care testing SOPs
· Maintain accurate, contemporaneous PMRs (ensuring appropriate transfer of information from INRStar to the PMR) in line with information governance and clinical safety standards
· Support audit, reporting and data collection activities for service evaluation and quality improvement
Quality & Safety
· Report incidents, near misses, equipment failures, or safety concerns in line with organisational policy
· Work within national guidance (e.g. NICE, MHRA) and local anticoagulation protocols

Maintain competence in PoC INR testing, including basic care and maintenance for the equipment, consumables and clinical supplies used within the service
Pay: £17.00-£19.00 per hour
