Full job description

Experience in primary care and an Independent Prescriber status is essential for this role. Please only apply if you meet this criteria. All other applications will be ignored.

We are specialist providers of clinical pharmacy services for PCNs and GP practices across the UK.

Clinical Support Solutions are looking to recruit several clinical pharmacists for primary care networks (PCNs) nationwide. We are looking for motivated and enthusiastic clinical pharmacists who have previous experience of working in primary care.

Full time and part time roles available with excellent rates of pay.

Location: UK Remote

Requirements for the role

The successful candidate(s) will have

  •  a Pharmacy Degree with at least 2 years post graduate experience
  •  an understanding and experience of working as a clinical pharmacist in primary care
  • an Independent Prescriber status
  •  excellent clinical and medication knowledge.
  •  experience of undertaking medication reviews.
  •  the commitment to continue to learn and develop.
  •  excellent inter-personal and patient facing communication skills.
  •  registered with the General Pharmaceutical Council.
  •  personal Professional Indemnity Insurance.

Job summary

The post holder will,

  • act within their professional boundaries and clinical competence
  •       work as part of a multi-disciplinary team and will take responsibility for areas of chronic disease management
  •       undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
  •       provide primary support to general practice staff with regards to prescription and medication queries.
  •       help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing,
  • provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
  •       have a key role in supporting delivery of the new Network Contract DES Service specifications.
