About the Role

The Children's Weight Management Practitioner will play a key role in delivering engaging and effective weight management and healthy lifestyle programmes for children and families across Durham. Working closely with schools, parents, school nurses, healthcare professionals, and community stakeholders, you will support participants to make positive behaviour changes through education, motivation, and ongoing support.

The role involves delivering group sessions, supporting referrals and engagement, maintaining accurate records, and contributing to the achievement of programme targets and outcomes. The successful candidate will be passionate about improving health and wellbeing, confident working with children and families, and committed to delivering high-quality services that make a real difference.

Key Responsibilities

• Deliver high-quality healthy lifestyle and weight management programmes for children, young people, and families, using the Morelife curriculum to support positive behaviour change and the achievement of SMART goals.
• Engage, support, and communicate with participants and stakeholders, including parents, schools, school nurses, healthcare professionals, community organisations, and referrers, ensuring effective recruitment, participation, and ongoing engagement throughout the programme.
• Maintain accurate records and contribute to service performance, including monitoring participant progress, collecting feedback, managing data in line with GDPR requirements, and supporting the achievement of contractual KPIs and service targets.
• Ensure safe, professional, and effective programme delivery by adhering to safeguarding and child protection policies, managing programme resources and equipment, and promoting the values, aims, and objectives of Morelife UK.
• Contribute to the continuous development and success of the service through attendance at meetings, training and supervision, working flexibly across community settings throughout Durham, and undertaking additional duties appropriate to the role as required.

Essential Qualifications & Experience

• Experience delivering behaviour change, healthy lifestyle, or weight management interventions, with the ability to support individuals in achieving positive health outcomes.
• Experience working with children, young people, and families, alongside knowledge of safeguarding practices and the ability to work sensitively with vulnerable individuals.
• Strong organisational, administrative, and IT skills, including experience coordinating tasks, maintaining accurate records, and using Microsoft Office applications such as Word, Excel, and Outlook.
• Excellent communication and interpersonal skills, with the ability to build effective relationships with participants, families, schools, healthcare professionals, and community stakeholders, together with a full UK driving licence and access to a vehicle for travel across Durham.
