Overview
We are seeking a highly organised and experienced Clinical Triage Manager to support the delivery of our specialist remote Tier 3 Weight Management Service through the Gro Health platform.
This is a non-clinical administrative role responsible for managing the referral and triage process for patients referred into the service. The postholder will ensure referrals are reviewed against agreed eligibility criteria, patient records are complete, and individuals are appropriately onboarded into the service pathway.
Working closely with clinicians, service managers, and external referral partners, the Clinical Triage Manager will play a key role in ensuring timely access to care, maintaining data quality, and supporting a positive patient experience.
The successful candidate will have experience working in a healthcare environment, excellent attention to detail, and a strong understanding of patient pathways, referral management, and clinical governance processes.
Key Responsibilities
Referral and Triage Management:
- Receive, review, and process referrals into the Tier 3 weight management service.
- Assess referrals against agreed service eligibility criteria and triage protocols.
- Identify missing information and liaise with referrers, GP practices, or patients to obtain required documentation.
- Ensure referrals are allocated to the appropriate pathway within agreed service timeframes.
- Escalate referrals requiring clinical review to appropriate members of the multidisciplinary team.
- Maintain accurate records of triage decisions and actions taken.
Patient Journey Coordination:
- Act as a key point of contact for patients during the referral and onboarding process.
- Support patients with registration and access to the digital platform.
- Coordinate appointments and assessments where required.
- Monitor patient progression through the onboarding pathway and resolve administrative barriers to access.
Quality and Governance
- Ensure all triage activities comply with organisational policies, clinical protocols, GDPR, and information governance requirements.
- Maintain accurate and auditable records within clinical and operational systems.
- Support service audits, reporting requirements, and quality improvement initiatives.
- Identify trends, risks, or process issues and escalate appropriately.
Stakeholder Communication:
- Build effective relationships with GP practices, Integrated Care Systems (ICSs), commissioners, and other referral partners.
- Communicate professionally with patients, carers, and healthcare professionals.
- Support the resolution of referral-related queries and complaints.
Service Improvement:
- Contribute to the development and refinement of referral pathways and triage processes.
- Assist in creating standard operating procedures (SOPs) and training materials.
- Support service mobilisation and expansion activities as required.
Essential Criteria
Experience:
- Experience working within a healthcare, NHS, community health, or specialist service setting.
- Experience managing referrals, waiting lists, patient pathways, or clinical administration processes.
- Experience interpreting and applying eligibility criteria or service protocols.
- Experience handling sensitive patient information confidentially and accurately.
Knowledge:
- Understanding of patient referral pathways and healthcare administration processes.
- Understanding of information governance, GDPR, and confidentiality requirements.
- Familiarity with clinical record systems and digital healthcare platforms.
- Awareness of multidisciplinary healthcare services.
Skills:
- Excellent organisational and prioritisation skills.
- Strong attention to detail and accuracy.
- Ability to manage a high-volume workload while meeting performance targets.
- Excellent written and verbal communication skills.
- Strong problem-solving and decision-making abilities.
- Proficient in Microsoft Office and digital systems.
Desirable Criteria
- Experience working within weight management, obesity services, diabetes services, public health, or long-term condition management.
- Experience working within a remote or digital healthcare service.
- Knowledge of Tier 3 weight management pathways.
- Experience using electronic patient record (EPR) systems and CRM platforms.
- Understanding of NHS referral management systems
Pay: £25,000.00-£28,000.00 per year
