Full job description
Keep primary care running smoothly, every day.
Care Navigator
At DHU Healthcare CIC, every role matters. We’re a not-for-profit team driven by compassion and respect, where your voice is heard and your growth truly matters. Join us and be part of something bigger. For you, for them, with us.
With us, care never stops.
At DHU Healthcare CIC, our Primary Care teams work at the heart of local communities, delivering high-quality, patient-centred services across the areas we serve. Based at Eyam Surgery, with flexibility to support Bradwell Surgery when required, you’ll play a vital role in ensuring patients receive timely access to the right care and a positive experience from the moment they contact or visit the practice.
Our teams of skilled clinicians and support professionals work together to provide responsive, professional NHS services. As a Care Navigator, you’ll be central to guiding patients to the most appropriate service, helping the practice run efficiently while always keeping patient wellbeing at the forefront.
As a Care Navigator you will:
- Be the welcoming, professional first point of contact for patients and visitors, both face to face and on the telephone.
- Open and close Eyam and Bradwell Surgery as required, ensuring the premises are safe and ready for service.
- Book appointments for patients with the clinical team and other services using the clinical system in place.
- Signpost patients to the most appropriate services, including community pharmacy, social prescribing, physiotherapy and other community-based support.
- Ensure messages are passed quickly and accurately to the correct member of the team via electronic systems or telephone, following surgery protocols.
- Contact patients with test results and other information on behalf of clinicians.
- Process repeat prescription requests and escalate queries appropriately.
- Monitor and respond to incoming emails in a timely and professional manner.
- Maintain tidy and welcoming reception and waiting areas, ensuring posters and information are current and appropriate.
- Receive and record patient payments where applicable, balancing takings at the end of each shift.
- Support daily operational tasks such as notifying the secretary of incoming post and preparing clinical rooms where required.
- Adhere to all surgery policies, including Health & Safety, safeguarding and confidentiality requirements.
- Provide flexible support to cover planned and unplanned staff absences across both sites.
- Always present a professional image, wearing the appropriate uniform and name badge.
You’ll bring excellent communication and interpersonal skills, strong organisational ability and confidence using IT and digital systems. You’ll be calm under pressure, able to multitask in a busy environment and committed to delivering a patient-focused, compassionate service.
For you, because great care starts with a supported team
We’ll support your health, happiness and future with benefits that put you first. From access to the NHS Pension Scheme (where applicable), to wellbeing support, flexible options and career development opportunities, we’re here to help you thrive at work and in life.
We foster a coaching culture with visible, supportive leadership and meaningful opportunities for progression within primary care.
For them, a place where you can be yourself
The best care comes from teams where everyone feels valued and supported. We welcome colleagues from all backgrounds and are committed to creating an inclusive workplace that reflects the communities we serve.
With us, your potential becomes progress
Development is part of our culture. From essential training and team learning to ongoing professional development, we’ll help you build confidence, capability and a rewarding long-term career in primary care.
We are committed to safeguarding and promoting the welfare of adults, children and young people and expect all colleagues to share this commitment. This role is subject to satisfactory pre-employment checks. Flexibility is required to meet service and rota needs, including covering annual leave and sickness where appropriate. Travel between sites will be required.
We’re proud to offer fulfilling roles with real opportunities for growth – plus training, wellbeing benefits and clear career pathways.
Ready to make the difference? We’d love to hear from you.
