Job Summary
The Care Navigator plays a crucial role in facilitating patient care by providing support and guidance to individuals navigating the healthcare system. This position requires a proactive individual with strong organisational skills and the ability to communicate effectively. The Care Navigator will assist patients in accessing necessary services, ensuring they receive the appropriate care in a timely manner.

Duties

  • Serve as the primary point of contact for patients, addressing inquiries and providing information regarding healthcare services.
  • Assist patients in scheduling appointments and coordinating care with various healthcare providers.
  • Maintain accurate patient records through data entry and clerical tasks, ensuring confidentiality and compliance with regulations.
  • Manage administrative tasks including filing, typing correspondence, and maintaining office supplies.
  • Demonstrate excellent telephone and face to face skills while communicating with patients, families, and healthcare professionals.
  • Collaborate with multidisciplinary teams to enhance patient care pathways and improve service delivery.

Requirements

  • Previous office experience is essential, with a preference for candidates who have administrative experience in a healthcare setting.
  • Proficient computer skills including familiarity with Microsoft Office.
  • Strong organisational skills with the ability to manage multiple tasks efficiently while maintaining attention to detail.
  • Excellent typing skills for accurate data entry and documentation purposes.
  • Demonstrated clerical experience that showcases your ability to handle administrative duties effectively.
