Care Coordinator (Preston)

As a Care Coordinator in Preston, you’ll be at the heart of making sure our clients receive the right care at the right time—every time. You’ll coordinate care visits, support care teams and maintain excellent communication with clients and their families. What you’ll be doing

In this role, you’ll combine empathy with organisation to help deliver person-centred care. Your key responsibilities will include:

  • Coordinating care schedules to ensure consistent, timely support for clients
  • Communicating clearly with clients, families and care staff—keeping everyone informed and reassured
  • Monitoring care delivery so service quality remains high and needs are met proactively
  • Handling day-to-day coordination tasks, including rostering, visit planning and ensuring coverage
  • Supporting recruitment and onboarding activity where required, helping new starters get settled
  • Escalating concerns appropriately and working with the wider team to resolve issues quickly and professionally
  • Keeping accurate records and maintaining compliance with company processes
  • Promoting a positive, respectful culture that reflects how care should feel—human, dignified and dependable
