Patient Pathway Coordinator (PPC)

Greater Lancashire Hospital

Role Overview

We are looking for an organised, proactive and detail-oriented Patient Pathway Coordinator to join our team.

Please note: This is a full time position, Monday to Friday 8am till 4pm. Shift pattern will be reviewed after the initial training period and may be changed to 9am to 5pm or 10am to 6pm.

The successful candidate will manage and coordinate patient pathways from referral through to discharge, ensuring a smooth, efficient process and an excellent patient experience.

Key Responsibilities

  • Coordinate patient referrals, appointments, diagnostics and follow-up care.
  • Ensure patient information and test results are accurately recorded and shared with relevant clinicians.
  • Monitor patient pathways to ensure timely and effective care.
  • Liaise with consultants, patients and internal teams to ensure smooth communication.
  • Identify and resolve issues to improve processes and patient experience.
  • Maintain accurate patient records and ensure confidentiality.

Skills & Experience

  • Previous experience in healthcare administration or appointment bookings.
  • Excellent IT skills, including Microsoft Office (Word, Excel and Outlook).
  • Experience using patient administration systems.
  • Excellent organisational skills and attention to detail.
  • Strong communication and interpersonal skills.
  • Ability to prioritise tasks, multitask and work under pressure.
  • Proactive, flexible and able to work independently or as part of a team.
  • A positive, problem-solving attitude with a focus on efficiency and patient care.

Job Type: Full-time

Pay: From £12.71 per hour

Expected hours: No less than 37.5 per week
