Job Summary

We are seeking a dedicated and organised Clinical Care Co-ordinator to join our domicilliary care team. The successful candidate will play a vital role in managing patient care coordination, ensuring seamless communication between patients and healthcare providers, and supporting clinical operations. This role offers an opportunity to contribute to high-quality patient care within a dynamic environment.

Duties

  • Support with the clinical face to face contact with clients, supporting personal care and medication administration.
  • Act as a liaison between clients, other healthcare professionals, and administrative staff to facilitate smooth communication.
  • Maintain accurate and up-to-date patient records in compliance with data protection regulations.
  • Working within an on-call rota with others, dealing with phone enquiries and communicating information or changes to cares on shift that day.
  • Assist with the preparation of clinical documentation and reports as required.
  • Provide administrative support by managing other carers at work that day, answering queries, and coordinating with external agencies.
  • Support the implementation of clinical protocols and procedures to enhance service delivery.
  • Utilise office experience to manage scheduling systems, databases, and other administrative tools effectively.
  • Contribute to continuous improvement initiatives within the clinical team.

Requirements

  • Strong organisational skills with the ability to prioritise tasks effectively.
  • Ability to problem solve and keep calm under pressure.
  • Excellent communication skills, both written and verbal, with a professional approach.
  • Ability to work independently as well as part of a multidisciplinary team.
  • Attention to detail in maintaining accurate records and documentation.
  • Compassionate attitude towards clients and colleagues alike.
