Care Coordinator – Sittingbourne Primary Care Network (PCN)

Responsible to: Social Prescribing Service Manager

Based: Hybrid role based remotely, at Involve Kent office based in Maidstone, PCN sites, and in GP practices: Memorial Medical Centre, The Grovehurst Surgery, The Medic Care Surgery, The Chestnuts Surgery, Green Porch Medical Centre, The Meads Medical Practice.

We are looking for a Care Coordinator to work in Sittingbourne PCN and use data, analysis and population health management tools to support primary care teams to provide proactive and coordinated care to patients. The focus will be on patients with complex long-term health conditions, frailty, and those at high risk of unplanned hospital admission. The Care Coordinator will support member practices to achieve key population health objectives, improve disease register management, and deliver health interventions.

You will need to be able to drive and have access to a vehicle for this role, as it involves community and home visits. The role includes providing regulated activity with adults, so employment is subject to an Enhanced DBS check.
