Job Overview
Patient Care Navigator (Clinical)
Join our dynamic healthcare team as a Patient Care Navigator (Clinical), where you will play a vital role in guiding patients through their healthcare journey with compassion, clarity, and efficiency. In this energetic position, you will serve as a key point of contact for patients, helping them access the services they need while coordinating care across multiple departments. Responsibilities
- Act as the primary point of contact for patients, providing guidance and support throughout their care process

- Maintain accurate patient records and documentation within electronic health record systems

- Clinical Training /Qualifications
