Job Overview
Patient Care Navigator (Clinical)
Join our dynamic healthcare team as a Patient Care Navigator (Clinical), where you will play a vital role in guiding patients through their healthcare journey with compassion, clarity, and efficiency. In this energetic position, you will serve as a key point of contact for patients, helping them access the services they need while coordinating care across multiple departments. Your proactive approach and excellent communication skills will ensure a seamless experience for patients, empowering them to take charge of their health with confidence. Responsibilities
- Act as the primary point of contact for patients, providing guidance and support throughout their care process
- Assist patients in scheduling appointments, follow-ups, and referrals to specialists or community resources
- Coordinate communication between patients and healthcare providers to ensure clarity and timely updates
- Maintain accurate patient records and documentation within electronic health record systems
- Provide information about treatment plans, medication instructions, and available support services
- Support administrative tasks such as managing office correspondence, data entry, and appointment reminders
- Foster a welcoming environment by addressing patient questions and concerns with professionalism and empathy
Qualifications
- Prior office experience is preferred to effectively manage administrative duties and patient interactions

- Excellent verbal and written communication skills to clearly convey information and build rapport with diverse patient populations
- Demonstrated ability to work independently while collaborating effectively with healthcare teams

- Clinical Training /Qualifications
