About the Role

As a Care Coordinator, you’ll be at the heart of our care team, helping to ensure clients receive safe, consistent and personalised care while supporting our carers and clinical team.

You will:

  • Coordinate and maintain care rotas up to six weeks in advance.
  • Build positive, trusting relationships with clients and carers.
  • Match carers to clients based on availability, preferences and skills.
  • Work closely with the clinical team and keep them updated on client needs.
  • Manage the on-call rota and provide out-of-hours support.
  • Conduct client home visits when required.
  • Support the recruitment and welcoming of new staff.
  • Answer calls and provide professional, friendly support.
  • Use digital care systems such as Birdie and Microsoft Office.
  • Contribute to a positive, supportive and welcoming office environment.
  • Take initiative, stay organised and ensure day-to-day operations run smoothly.
  • Full UK driving licence and access to own transport required.
