About Us

Prevail Healthcare is a CQC-registered provider of high-quality domiciliary and live-in care across Norfolk. We are a friendly, family-orientated organisation known for delivering person-centred care and building long-lasting relationships with both our clients and carers. Due to maternity leave, we are seeking a reliable and organised Care Coordinator To join our office team.

The Role

The Care Coordinator is a key part of our operations team, responsible for ensuring care is delivered smoothly and safely in line with each client’s care plan and preferences.

Key Responsibilities:

  • Scheduling and coordinating home care and live-in care visits
  • Ensuring continuity of care for clients through effective rota planning
  • Managing incoming queries and last-minute changes to care schedules
  • Supporting recruitment, onboarding, and compliance processes
  • Maintaining accurate records in line with CQC requirements
  • Liaising with local authorities, healthcare professionals, clients, and families
  • Participating in the on-call rota (additional payment provided for on-call duties)

What We’re Looking For

  • Previous experience in a care coordination or scheduling role is desirable
  • Excellent organisational and time management skills
  • Strong communication and interpersonal abilities
  • Confident using Microsoft Office and rostering systems (training provided)
  • A team player with a proactive attitude and the ability to work under pressure
  • Knowledge of domiciliary care services and an understanding of CQC standards
