Full job description

The successful candidate will be offered a fixed term to cover Maternity Leave for 1 year with a view to extend.

Care co-ordinators work with people to build trusting relationships and listen to what matters to them. They work with a range of people, particularly those with long-term conditions, multiple long-term conditions, and people living with or at risk of frailty. They help people co-ordinate and navigate their care across the health and care system and can support people to become more active in their own health and care.

Main duties of the job

Administration:

Develop a working knowledge of the GP and Network contract, the Quality Outcomes Framework/Impact and Investment.

To support the Newton and Haydock Community in signposting to appropriate services both internally and externally in the community

To work with community, social, educational and medical services & breaking down the barriers between them.

Relationship Management

To work as a flexible member of the PCNs, providing support to other team members when necessary

To take an active role in supporting development and embedding of the PCNs culture, values and reputation as providers of high-quality services

To support in encourage all staff to collaborate through sharing information and intelligence across different activities

Build good relationships with all stakeholders involved in the PCN

Liaise with other NHS providers and agencies in support of the work of the Network. Work alongside other PCN team members.

General Competencies:

To be responsible for planning and organising own workload, continually reprioritising your own work to meet changing local & national targets and deadlines

To act as a point of contact with regard to the delivery of Networks' projects/schemes

Supporting with the management of contractual requirements on behalf of the network.

Newton and Haydock Primary Care Network consists of 4 NHS GP practices in Newton-le-Willows and Haydock, across several sites. The Network enables NHS staff to work across the practices to help deliver services to 50,000 patients.

Job Description

The successful Care co-ordinator will be proactive in helping to deliver projects within the PCN. The post holder will be expected to liaise with both clinical and non-clinical staff in providing patient care via project support.

Collaborating closely with schools within Care Communities to provide support for children with low attendance, and offering guidance to families regarding available community and medical services that benefit both the child and the entire family.

Additionally, supporting St Helens Place in the seamless integration of Neighbourhood Health initiatives. St Helens is a pioneering site for Neighbourhoods, one of 43 locations across England participating in this program.

The successful candidate will assist patients in managing their long-term conditions and coordinate educational opportunities to enhance their understanding of their health. This role involves working collaboratively with secondary care, community services, and public health entities to develop and implement new care pathways.

We are currently developing a prevention-focused model that emphasizes patient education on early detection and the prevention of health conditions. The goal is to facilitate earlier intervention, thereby reducing wait times in both primary and secondary care settings.

Where necessary, the post holder will be required to book clinic rooms, or meeting rooms, both clinical and non-clinical colleagues from any of the GP practices within the PCN catchment area, or PCN employees.

The ideal candidate will need to be proactive, have a passion for primary care, and have a proven track record in administrative coordination. The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day coordination.

Pay: £24,420.00-£27,000.00 per year
