Job Summary

The successful candidate will be responsible for undertaking a wide range of duties in support of the multidisciplinary team. Duties can include but are not limited to triaging the Blinx requests that come through to the practice, overseeing the appointment book, liaising with the triage GP and other members of the team. Primary Responsibilities The following are the core responsibilities of a care navigator, there may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Triage the main Blinx list
  • Oversee the appointment book to ensure no appointment slots are unfilled
  • Liaise with pts to inform them of what is happening
  • Liaise with the triage GP
  • Manage the triage admin list
  • Manage the chronic disease & specialist clinic lists
  • Manage the 111 list
  • Re-allocate appointments for any absent GPs

We are looking to recruit the right person to join our friendly and supportive team. Applicants should have excellent verbal and written communication skills, be hard working and have the ability to multitask. Having excellent customer care and patience are essential skills for this role. We are a GMS teaching practice located in central West Kirby, caring for over 19000 patients and have an elderly patient demographic. The practice is made up of 7 GP partners, 8 salaried GPs, 3 ANPs, 6 Practice Nurses and 3 HCAs. We are involved with Liverpool University Medical School and have a teaching ethos hosting medical students, F2s and train ST3s. Our mission is to deliver safe, compassionate and high-quality healthcare through an empowered and collaborative team. We are dedicated to providing accessible and innovative services that respond to the needs of our community, supporting every patient on their journey to better health and wellbeing.

We provide plenty of tea/coffee and toast. We also like to recognise and celebrate staff special occasions. Staff are required to dress appropriately for their role. Administrative staff will be provided with a uniform.
