Job Summary

We are seeking a dedicated and detail-oriented Pharmacist-IP to join our healthcare team. In this vital role, you will be responsible for providing expert medication management and patient care within an inpatient setting. Your primary focus will be ensuring safe and effective medication use, supporting clinical teams, and delivering exceptional patient-centered services.

Job Duties

• Provide safe, effective and patient-centred clinical consultations as a Pharmacist Independent Prescriber.
• Conduct face-to-face patient consultations, taking a comprehensive clinical history, assessing symptoms and making appropriate clinical decisions.
• Assess, diagnose and manage a wide range of minor ailments and common clinical conditions, providing appropriate treatment, self-care advice and safety-netting.
• Utilise previous GP practice experience to assess patients holistically and manage presentations within the scope of professional competence.
• Prescribe appropriate medicines in accordance with clinical guidelines, local protocols, and professional and legal requirements.
• Recognise red flags and patients requiring escalation, urgent assessment, referral or onward management.
• Provide clear safety-netting advice and ensure patients understand when and where to seek further medical attention.
• Undertake clinical assessments and document consultations accurately and contemporaneously in the appropriate clinical record system.
• Work collaboratively with GPs, practice staff, community pharmacists and other healthcare professionals to ensure continuity and coordination of patient care.
• Review patients' medication where appropriate and identify potential drug interactions, contraindications, adverse effects and prescribing concerns.
• Promote appropriate use of medicines, adherence to treatment and patient understanding of prescribed therapy.
• Maintain confidentiality and comply with GDPR, information governance, safeguarding and professional standards.
• Keep clinical knowledge and prescribing practice up to date through continuing professional development (CPD).
• Participate in clinical governance, audit, quality improvement and service development activities where required.
• Deliver a professional, compassionate and inclusive service, ensuring patients feel listened to and involved in decisions about their care.

Essential Skills & Experience

• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Qualified and practising as a Pharmacist Independent Prescriber, with an appropriate prescribing qualification.
• Previous experience working within a GP practice / primary care setting.
• Strong clinical assessment and consultation skills, particularly in face-to-face patient consultations.
• Demonstrable expertise in assessing and managing minor ailments and common acute conditions.
• Ability to take a thorough clinical history, assess symptoms, identify differential diagnoses and formulate an appropriate management plan.
• Sound knowledge of prescribing principles, medicines optimisation, contraindications, interactions and adverse drug reactions.
• Ability to recognise red flags and clinical deterioration and make appropriate referrals or escalate care when necessary.
• Excellent communication and interpersonal skills, with the ability to explain clinical information clearly to patients.
• Strong clinical decision-making and professional judgement, while recognising personal limitations and seeking appropriate support when required.
• Ability to provide effective safety-netting and self-care advice.
• Accurate and confident use of electronic clinical record and prescribing systems.
• Good understanding of safeguarding, confidentiality, information governance and relevant UK healthcare legislation.
• Commitment to patient-centred care, professional accountability and continuous learning.
• Ability to work effectively both independently and as part of a multidisciplinary primary care team.
• A professional, approachable and empathetic manner when dealing with patients of different ages and backgrounds.

Desirable Skills

• Experience managing a broad range of presentations commonly encountered in primary care.
• Experience working within NHS primary care or an equivalent UK clinical environment.
• Familiarity with local and national prescribing guidelines and primary care pathways.
• Experience with clinical audit, quality improvement or service development.
• Experience using GP clinical systems and electronic prescribing platforms.

Pay: £45,000.00-£60,000.00 per year
