Triage Practitioner

Location: Birmingham ( Telephone, Video Delivery & Face-to-Face)
Salary: £24,500per annum
Contract: Full-Time, Permanent

Due to continued growth, we are seeking a motivated and compassionate Triage Practitioner to join our team.

The Role

As a Triage Practitioner, you will play a vital role in ensuring clients receive the right support at the right time. You will conduct comprehensive mental health assessments, risk assessments, and treatment suitability screenings to determine the most appropriate care pathway for individuals accessing our service.

You will work closely with our clinical team, including PWPs, CBT Therapists, Counsellors, and Clinical Supervisors, to ensure safe and effective client care.

Key Responsibilities

Clinical Assessment & Triage

  • Conduct comprehensive mental health assessments via face-to-face, telephone, and video appointments.
  • Gather relevant information regarding presenting difficulties, mental health history, risk factors, and treatment goals.
  • Complete robust risk assessments, including assessment of:
  • Suicide risk
  • Self-harm
  • Safeguarding concerns
  • Risk to others
  • Domestic abuse
  • Substance misuse
  • Determine suitability for treatment and allocate clients to the most appropriate pathway.
  • Identify clients requiring urgent intervention and escalate concerns appropriately.
  • Provide signposting and onward referrals where appropriate.

Clinical Administration

  • Maintain accurate and timely clinical records using IAPTUS.
  • Ensure all assessment documentation is completed to a high standard.
  • Record outcome measures and relevant clinical information in line with service requirements.
  • Adhere to information governance, confidentiality, and data protection standards.

Service Delivery

  • Manage a caseload of assessments and triage appointments.
  • Work collaboratively with clinicians and management teams.
  • Attend clinical supervision, case discussions, and team meetings.
  • Contribute to service improvement initiatives and quality standards.

What We’re Looking For

Essential Criteria

  • Bachelor’s Degree (2:1 or above preferred) in:
  • Psychology
  • Mental Health Nursing
  • Counselling
  • Health & Social Care
  • Or a related discipline
  • Experience working within a mental health service.
  • Experience conducting mental health assessments and triage appointments.
  • Experience completing risk assessments and managing risk appropriately.
  • Knowledge of common mental health conditions including:
  • Depression
  • Anxiety Disorders
  • Panic Disorder
  • OCD
  • PTSD
  • Health Anxiety
  • Strong understanding of safeguarding procedures.
  • Excellent verbal and written communication skills.
  • Ability to work independently whilst maintaining high clinical standards.
  • Strong organisational and time management skills.
  • Proficient IT skills and confidence using electronic patient record systems.

Desirable Criteria

  • Experience using IAPTUS.
  • Experience within NHS Talking Therapies services.
  • Knowledge of stepped-care models.
  • Experience working with diverse communities.
  • Previous experience in Psychological Wellbeing Practitioner (PWP) services.
  • Understanding of NHS Talking Therapies outcome measures.
