We are seeking an exceptional Programme Lead Health & Care to lead some of our most important health and care transformation programmes in Merton.

This high-profile role will drive the development and delivery of integrated health and care arrangements across Merton, helping to improve outcomes, reduce health inequalities, and support residents to live healthier, more independent lives.

Working across the Council, NHS partners, Primary Care Networks, voluntary sector organisations and the wider Integrated Care System, you will play a pivotal role in designing and embedding neighbourhood-based models of care and supporting the continued development of Integrated Neighbourhood Teams.

You will also provide strategic leadership for Merton's Better Care Fund programme, ensuring strong governance, delivery, performance and impact.

About the Role

As Programme Lead Health & Care, you will:

• Lead the development and implementation of integrated health and care programmes at place and neighbourhood level.
• Provide strategic leadership for the Better Care Fund, ensuring delivery against national and local priorities.
• Work with partners to design and embed Integrated Neighbourhood Teams and multidisciplinary models of care.
• Drive service transformation that promotes prevention, independence and person-centred support.
• Develop strong relationships across the NHS, local government, primary care, voluntary and community sectors.
• Lead programme governance, assurance, performance management and reporting arrangements.
• Influence senior leaders, boards, elected members and system partners to achieve shared outcomes.
• Support the development of innovative solutions that improve outcomes and reduce demand on acute services.
• Champion approaches that address health inequalities and improve population health.

About You

We're looking for a strategic and collaborative leader with a proven record of delivering complex transformation programmes across organisational boundaries.

You will have:

• Significant senior leadership experience within health, social care, local government or a related public sector environment.
• A successful track record of delivering major transformation and improvement programmes.
• Experience leading partnership working across NHS organisations, local authorities and wider system partners.
• Strong knowledge of Integrated Care Systems, neighbourhood health models and place-based working.
• Experience of leading Better Care Fund programmes or similar pooled budget arrangements.
• A deep understanding of health and care integration, prevention and population health approaches.
• Excellent programme, performance and financial management skills.
• Strong influencing, negotiation and stakeholder engagement abilities.
• Experience of developing services that improve outcomes and tackle inequalities.
• Degree-level qualification or equivalent relevant experience.

We offer flexible working, with a requirement for 3 days in the office/ in the local community.
