Ensure effective collaboration and engagement across the Trust, ICB and West Midlands Cancer Alliance to implement the key measures for personlised cancer care. Work with major stakeholders , CNS’s, cancer teams, Navigators, primary, secondary and tertiary services across acute and primary care.

Support the ongoing development of the Living with and beyond cancer service (LWBC) - setting and measuring objectives towards successful implementation at Walsall Manor hospital, working with all key leads to increase the amount of PCSP that are produced in conjunction with HNAs, treatment summary and opportunity to increase PSFU pathways.

The post holder will work as part of the Personalised Cancer Care Support Team working across primary, secondary and tertiary sector services. The post holder will lead on the delivery of the secondary care aspects of the Living with and Beyond Cancer LWBC programme. These include;

• Embedding Holistic Need Assessments (HNA), Person support care plans (PSCP)
• Treatment Summaries
• Delivery of the Health and Wellbeing Events
• Person centred Follow- Up (PSFU).

The aim of this post is to support the Trust Lead Cancer Nurse and its Cancer MDT’s in the delivery of the cancer survivorship agenda by developing trust wide strategies and new models of working for patients living with and beyond cancer. This role will require effective communication with wider stakeholders including Leads and Managers, Multidisciplinary Teams MDT, Macmillan Partnership Quality Leads and patients.

The post holder will be a member of the cancer services team, assist the lead on embedding survivorship and be responsible for the further development and implementation of the cancer patient survivorship programme at Walsall Healthcare NHS Trust. They, along with the lead will provide specialist advice and guidance on new models of working with patients living with and beyond cancer to members of the trusts multi-disciplinary team’s patients and other health practitioners in primary and secondary care.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. A new Urgent and Emergency Care Centre, opened in March 2023, has significantly improved emergency care facilities and capacity, providing almost 5,000 square metres of additional clinical space.

Key Responsibilities

Trusts Cancer Survivorship Agenda:
Support the Lead on the development of a LWBC programme providing options for future sustainability of developing model(s) of care for patients living with and beyond cancer.
Identify ways that service redesign may be used in the follow-up pathway to reduce future presentation to emergency care/GP services and / or as a means of reducing patient follow-up appointments (i.e. risk stratified pathways) and evaluate the impact this may have on both trust and community resources.
Work with the lead, by Liaising and influencing Clinical Commissioning Groups and Senior Healthcare Practitioners in promoting and raising the awareness of new models of cancer care follow-up

Health Needs Assessment and Care Planning

Attend the Trusts Champion HNA Steering group to assist in planning and implementing strategies for the delivery of Holistic Needs Assessments and Care Planning to patients along site-specific MDT cancer pathways
Work with the Trusts Cancer and IM&T Services to identify methods for capturing and reporting of HNA and care plans entered onto the Somerset Cancer Registry and Trusts EPR
Ensure the HNA and Care Planning process is effectively promoted and supported within the trust to both staff and patients
Along with the lead Identify and action plan to resolve any issues raised by the Champion group arising from HNA and Care Planning implementation, highlighting and reporting areas of concern to the Survivorship Steering Group and Trusts Locality Cancer Action Team
Deliver effective methods to evaluate and monitor HNA and care planning progress and ensuring these are made available to trust wide key stakeholders through 1/4ly updates.
Identify training and personal development needs in support of the delivery of HNA ensuring CNS’s’ and the Macmillan Cancer Care Navigators are equipped to appropriate levels of competency.

Work with identified cancer MDT’s:
To provide comprehensive project management support to the MDT’s in planning, embedding and reporting the implementation of Holistic Needs Assessment HNA and Treatment Summaries, supporting clinicians to deliver HNA project outcomes that enable improved quality
To establish Health and Wellbeing events as part of routine follow-up and as a gateway to a range of self-management options
In implementing a timetable of Living Well self- management courses accessible from across the care pathway
To assist in producing a Community Resource File listing key organisations available to support wellbeing events and liaising with these organisations to better understand their services and accessibility / availability criteria
To embed End of Treatment Summaries to support liaison between primary and specialist care professionals

Communication strategy and awareness:
Develop a communication strategy to raise awareness among patients and healthcare providers of any new survivorship initiatives and outcomes
Develop and disseminate a range of information and support materials related to cancer survivorship in a variety of media
Prepare reports as required on project development and service evaluations for dissemination / presentation at local, regional and national events, along with the help of the lead
Disseminate findings of the project to the wider community of people involved in the care of cancer patients
Promote models of care and evaluation processes developed through the project to other tumour sites across the Trust
Work in partnership with all clinical and non-clinical staff within cancer services to achieve a seamless pattern of care for the individual patient

Professional Responsibilities

To assist the lead on the development of Trust/Network clinical policies, protocols and new pathways to support survivorship
Participate in Regional and National Group meetings representing Walsall Healthcare NHS Trust.
To act as a specialist in the field by keeping up to date by reviewing latest research/literature and attending relevant conferences/courses
To be an active member of the Trusts Cancer Nurse Specialist Meeting, in promoting the survivorship agenda and undertaking activities to take cancer survivorship nursing forward in the Trust.

Maintain own professional development and competencies through attending statutory study days as well as relevant courses to the service.
Attend regular Clinical Supervision sessions for personal and clinical support
Maintain clear and concise records / correspondence / files
Attend and keep up to date with the Trusts mandatory training requirements

Ensure Trust policies, procedures and guidelines/protocols are known and followed
Attend and participate in Departmental, directorate and Trust meetings as required
Participate with other members of the team in preparation for annual peer review
Participate in local and regional multidisciplinary meetings when required
To behave in a manner that secures the security of National Health property and resources
To safeguard at all times the confidentiality of information in relation to patients and staff
To follow risk management procedures at all times
To take personal responsibility for risk management in respect of self and colleagues and report any identified potential hazard to line manager

Demonstrate a good understanding and knowledge of local and national guidance and targets that impact the project e.g. NSF (National Service Framework), NICE (National Institute of clinical excellence) guidance, Department of Health initiatives etc.
Participate in planning and implementing changes within the area of care and responsibility
Contribute and participate in the development of local guidelines, protocols and standards.

Utilising Information

Devise and provide improvements to current management information, analysing, reporting and suggesting procedures to enhance decision- making processes.
Ensure timely and accurate information analysis and reporting to management on agreed areas of work
Undertake information searches using available data bases and resources to support MDTs
Understand the responsibility of self and others regarding the Freedom of Information Act
Collate, analyse and present clinical data and information to illustrate and explain data within general practice.

Learning and Development

Scope the training needs across all sectors and develop/deliver a continuous training package on Living with and Beyond Cancer i.e. face to face, e-learning, conferences
Disseminate learning and information gained to Primary Care, Third sector and secondary care teams in order to share good practice and inform others about current and future developments
Assess own learning needs and undertake learning as appropriate
Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information
Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.
Actively engages with Macmillan Cancer Support to contribute expertise and experience and support the Macmillan Corporate Strategy.
