An exciting opportunity has arisen to join the South London Partnership (SLP) Complex Care Programme, a partnership between Oxleas, South West London and St George’s, and South London and Maudsley NHS Mental Health Trusts.

This is an exciting opportunity for an experienced health or social care professional to join a newly established team and help shape how complex care is commissioned and delivered across South West London.

We are looking for someone with extensive experience of working with people with complex mental health needs, learning disability, autism and/or older people, who can work autonomously, make sound decisions and build effective relationships across organisations.

Main duties of the role

As part of this new team, you will have the opportunity to shape working practices and contribute to the continued development of the Complex Care Programme.

The main duties will include:

• Lead placement management – screen referrals, undertake assessments, attend funding panels, review placements and negotiate funding arrangements with NHS and Local Authority partners.
• Support transformation and reduce inpatient care – undertake commissioner visits, coordinate and chair Care (Education) and Treatment Reviews (CETRs), and support discharge and step-down planning.
• Coordinate Dynamic Support Registers – work with health, Local Authority and partner organisations to identify and support people at risk of hospital admission or placement breakdown.
• Support commissioning and service development – review urgent requests, contribute to bespoke care packages and develop innovative accommodation and support solutions that enable people to live closer to home.
• Lead improvement activity – contribute to project planning, implementation and evaluation, using data and evidence to improve outcomes, quality and value for money.

You will need to be confident working independently, managing competing priorities and making decisions within tight and changing timescales. Strong communication, negotiation and partnership-working skills are essential.

What you’ll need

• A relevant health or social care qualification, with Master's-level education or equivalent senior-level experience.
• Extensive experience working with people with complex mental health needs, learning disability, autism and/or older people.
• Knowledge of relevant legislation, guidance and policy frameworks.
• Experience of commissioning, placement management, project management or service development.
• Strong communication, negotiation, analytical and decision-making skills, with the ability to influence across organisations.
• Ability to work autonomously, manage complex situations and meet tight timescales.
• Commitment to person-centred care, equality, diversity, belonging and inclusion.

Desirable:

• Experience of CETRs and/or Dynamic Support Registers.
• Experience of service redesign, transformation or health and social care commissioning.
• Knowledge of Court of Protection or Deprivation of Liberty Safeguards processes.
