About Eeze Care London
Eeze Care London is a specialist provider of Supported Living services for adults with Learning Disabilities, Autism, Mental Health needs, Physical Disabilities, and complex support requirements. We are committed to delivering person-centered, outcome-focused services that promote independence, choice, dignity, and community inclusion.
We are seeking an experienced and motivated Assessment & Transitions Lead to oversee the end-to-end referral, assessment, commissioning, and transition process, ensuring individuals receive safe, high-quality, and well-planned support from their initial referral through to successful placement and ongoing integration into our services.
Role Purpose
The Assessment & Transitions Lead will be responsible to support our Head of Service for managing the complete referral-to-placement pathway across Eeze Care London services. This includes reviewing referrals, undertaking comprehensive assessments, developing support recommendations, liaising with commissioners and multidisciplinary teams, negotiating funding arrangements, coordinating placement transitions, and ensuring smooth mobilization of new services.
The post holder will act as the primary point of contact for local authorities, Integrated Care Boards (ICBs), commissioners, families, advocates, and internal operational teams throughout the assessment and transition journey along with our Directors and Head of Service. The post holder is expected to act swiftly and professionally on instructions issued by directors and head of service, recognizing that delays in assessment, commissioning, transitioning or operational actions may adversely affect placement opportunities and organizational growth.
Key ResponsibilitiesAssessment and Referral Management
- Manage all incoming referrals from commissioners, local authorities, NHS partners, and other stakeholders.
- Review referral information and identify gaps requiring further assessment or clarification.
- Undertake comprehensive person-centered assessments for individuals with:
- Learning Disabilities
- Autism Spectrum Conditions
- Mental Health Needs
- Physical Disabilities
- Complex Behavioural Support Needs
- Forensic Histories
- Dual Diagnoses
- Conduct assessments remotely and face-to-face across London and surrounding areas.
- Evaluate risks, strengths, support needs, communication requirements, and environmental considerations.
- Produce detailed, evidence-based assessment reports and placement recommendations.
- Ensure assessments comply with Care Act principles, Mental Capacity Act requirements, safeguarding legislation, and CQC expectations.
Commissioning and Business Development Responsibilities
- Develop strong professional relationships with commissioners, brokerage teams, social workers, caremanagers, and health professionals.
- Represent Eeze Care at commissioning meetings, placement panels, provider forums, and review meetings.
- Prepare and submit high-quality service proposals, support plans, and costed packages.
- Work collaboratively with commissioners to negotiate funding arrangements and placement agreements.
- Support occupancy growth across supported living services by identifying suitable referral opportunities.
- Monitor referral pipelines and provide regular reports on conversion rates and placement opportunities.
- Ensure proposed packages are financially viable and aligned with organisational objectives.
Support our Transition and Mobilisation Leadership
- Lead and coordinate all transitions into Eeze Care services along with Head of Service.
- Develop comprehensive transition plans for each new placement.
- Coordinate pre-admission meetings involving:
- Individuals
- Families
- Advocates
- Commissioners
- Clinical Professionals
- Operational/ Housing Teams
- Conduct compatibility assessments and environmental suitability reviews.
- Ensure all documentation, assessments, risk management plans, and support arrangements are completed prior to admission.
- Coordinate phased transitions where required with the support of initial transition support Plan.
- Monitor placement stability during the initial transition period and implement interventions where necessary.
- Ensure smooth handover to operational and service management teams.
Quality, Compliance and Risk Management
- Ensure all assessment and transition activities meet regulatory and contractual requirements.
- Maintain accurate records using company systems and referral tracking processes.
- Identify and escalate safeguarding concerns, risks, and placement suitability issues.
- Contribute to service development, quality improvement initiatives, and best practice standards.
- Support our Head of Service for the preparation for CQC inspections and commissioner audits relating to admissions, transitions, QA and assessment processes.
Stakeholder Engagement
- Act as a professional ambassador for Eeze Care London.
- Develop positive working relationships with:
- Local Authorities

- Ensure individuals and families experience a compassionate, transparent, and responsive assessment process.
Person SpecificationEssential
- Significant experience within Health and Social Care.
- Experience undertaking assessments for individuals with complex support needs.
- Strong understanding of Supported Living services.
- Knowledge of:
- Care Act 2014

- Positive Behaviour Support
- Autism and Learning Disability best practice
- CQC Regulatory Framework
- Experience working with commissioners and placement teams.
- Excellent report writing and assessment skills.
- Strong negotiation and stakeholder management abilities.
- Ability to manage multiple referrals and competing priorities.
- Full UK Driving Licence and willingness to travel.
Desirable
- Professional qualification in Social Work, Nursing, Psychology, Occupational Therapy, Health & Social Care, or a related discipline.
- Experience in commissioning, business development, or service mobilization.
- Knowledge of supported living housing pathways and accommodation-based services.
- Experience supporting complex transitions from hospital, residential care, family homes, or crisis placements.
