Are you passionate about helping people live independently, with dignity and choice?

We’re looking for caring, adaptable individuals to join our team as Multi-site practitioners, supporting people across our Extra Care Housing (ECH) and Learning Disability (LD) services.

This is more than a support role — you’ll positively impact lives, develop your skills, and help shape high-quality care through coaching and best practice.

What You’ll Be Doing

  • Providing compassionate, person-centred care and support
  • Supporting tenants with daily living, emotional wellbeing, and independence
  • Working across services to ensure consistent, high-quality care
  • Observing practice and helping maintain excellent standards
  • Supporting colleagues through coaching, guidance, and role modelling
  • Contributing to support plan reviews and improvements
  • Regular liaison and feedback with locality Senior teams
  • Keeping accurate records using digital systems
  • Promoting dignity, respect, and choice at all times
